It has also been found out that this trauma experienced through FGM stays with women forever. #endcuttinggirls</li> <li>Sexual phobia: women who have undergone FGM are more likely to experience painful intercourse, reduced sexual satisfaction and sexual desire. Women may also experience embarrassment about intimacy and more difficulty reaching orgasm. This is likely to adversely affect marital relationships, with the women bearing the blame for such occurrences. #endcuttinggirls</li> <li>Some survivors experience a sense of betrayal by someone emotionally close to them because in many cases, girls are held down by their relatives while FGM is carried out. This could in turn lead to feelings of resentment and withdrawal from such persons. #endcuttinggirls</li> <li>Flashbacks and sleep disorders: Girls who experienced FGM at relatively older ages are likely to become emotionally unstable at intervals when they read about FGM or remember the process. Images of the scenario are always fresh in their memories leading to inability to concentrate, behavioural and sleep disorders. #endcuttinggirls</li> <li>Post-Traumatic Stress Disorders (PTSD): FGM is very likely to cause various emotional disturbances which could lead to psychiatric disorders such as PTSD. This could also associate with memory dysfunction. #endcuttinggirls</li> <li>The effects of FGM/C stem beyond the medical effects. We also have Psychological and emotional effects which are rarely considered. #endcuttinggirls</li> <li>A lady who has undergone female genital mutilation is likely to experience some psychological trauma. Some of which include; Post-Traumatic Stress Disorder (PTSD): this is mostly common amongst ladies who can remember their FGM experience. The shock tends to come back to them every time they have a memory of it. #endcuttinggirls</li> <li>Ladies who have undergone FGM tend to have the same level of shock as adult who were subjected to Child Abuse #endcuttinggirls @endcuttinggirls</li> <li>Mood swings: as a lady who has been mutilated reads and learns more about what has been done and it’s possible effects, her mood start to change. From a feeling of hope to hopelessness, etc. #endcuttinggirls @endcuttinggirls</li> <li>Feeling of worthlessness: a number of people who have not been properly educated on FGM/C related issues tend to make careless remarks as regards the experience and those who have undergone it. These remarks might leave an FGM survivor feeling worthless. As though, a vital part of her has been taken away. #endcuttinggirls</li> <li>Drawn and untrustful: survivors of FGM/C tend to be drawn and keep to themselves and have a hard time trusting people. This could be due to the fact that those they trusted to protect them have unknowingly inflicted pain and a scar. #endcuttinggirls @endcuttinggirls</li> <li>Anger: the anger survivors feel towards the cutters and other family members that were involved in the process could, if not attended to, spread and generally make such a person develop anger control issues. #endcuttinggirls @endcuttinggirls</li> <li>Feeling of guilt, shame and fear: a lady who has been mutilated if not properly educated on the issue can go about carry a feeling of guilt, shame and fear. #endcuttinggirls @endcuttinggirls</li> <li>Sexual Phobia: this experience can lead to painful sex and cause the lady to develop a sort of phobia to sex. This could in turn take its toll on their spousal relationships. #endcuttinggirls @endcuttinggirls</li> <li>Depression and suicidal ideas: often times ladies who have been mutilated and have not had a chance to discuss this experience with specialists or anyone at all, tend to fall into depression which if not controlled, could birth suicidal thoughts. #endcuttinggirls @endcuttinggirls</li> <li>Understanding that FGM/C in some culture is a norm necessitates the need for partnership with relevant stakeholders. #endcuttinggirls @endcuttinggirls</li> <li>Also, identifying the right stakeholders and engaging them successfully is pivotal to #endcuttinggirls as it sets the tone for the partnership that can help reverse this trend @endcuttinggirls</li> <li>Asides traditional rulers and community leaders, TBAs are major stakeholders that cannot be left behind in achieving an FGM/C free generation. #endcuttinggirls</li> <li>36: Traditional birth attendants are women and men in the communities and villages who help pregnant women deliver their babies, and at most times, gain knowledge of the practice from their fore-fathers. #endcuttinggirls </li> <li>37: they are the first port of call for health issues relating to women and children in rural setting #endcuttinggirls @endcuttinggirls </li> <li>38: TBAs form a major number of the local performers (circumcisers) of FGM/C in their localities #endcuttinggirls @endcuttinggirls </li> <li>39: The role of the TBAs in providing available health care to pregnant and non-pregnant women in the communities cannot be overemphasized. #endcuttinggirls @endcuttinggirls </li> <li>40: The circle of TBAs remain one through which can speed up our reach and aid our goals to #endcuttinggirls @endcuttinggirls </li> <li>41: Partnering and engaging TBAs would be effective when we clearly define how they can help to implement FGM/C abandonment in their various local communities. #endcuttinggirls @endcuttinggirls </li> <li>42: we can engage them by doing an awareness program for TBAs, highlight the complications of FGM/C and encourage them to become advocates of FGM/C abandonment within and outside their communities. #endcuttinggirls @endcuttinggirls </li> <li>43: TBAs can function in line with their already-defined roles as advocates and law enforcers. #endcuttinggirls @endcuttinggirls </li> <li>44: As custodians of culture and highly-revered people in their communities, when TBAs become advocates of FGM/C abandonment, it gives the message “divine credibility” #endcuttinggirls @endcuttinggirls </li> <li>45: Being advocates erases the doubt of whether FGM/C is still acceptable in such communities. #endcuttinggirls @endcuttinggirls </li> <li>46: They will also lead discussions in the community which helps to banish the culture of silence around the practice #endcuttinggirls @endcuttinggirls </li> <li>47: TBAs can also share experiences and events that have occurred in their line of practice about FGM/C. #endcuttinggirls @endcuttinggirls </li> <li>47b: In 2016, according to a report in Sun newspaper, TBAs practitioners in Ebonyi state made open confessions and dropped their blades with which they cut the genitals of female children and embraced the call against FGM #endcuttinggirls @endcuttinggirls </li> <li>47c: This is a clear sign that when the TBAs are collaborated on this, it will speed up the abandonment of FGM/C #endcuttinggirls @endcuttinggirls </li> <li>48: According to Mrs Gloria Okorie a TBA from IVO in Ebonyi state, ““I thought I was doing female children well when I was cutting their female organ after birth. I thought doing that will make them complete and bear children when they grow up” #endcuttinggirls @endcuttinggirls </li> <li>49: “I have been telling people that I no longer do it. Even people are still coming to me to circumcise their female children and I use to tell them no, I don’t do it anymore. The following are complications of FGM/C</p> <p> </p> <h2>Short term complications</h2> <h3>Haemorrhage</h3> <p>Haemorrhage is one of the most common complications of FGM, as excision of the clitoris involves cutting across the high pressure clitoral artery and attempts to stop bleeding may not be effective. Acute extensive bleeding can lead to haemorrhagic shock or even sudden death in the case of cataclysmic haemorrhage. </p> <h3>Shock</h3> <p>Shock may occur because of blood loss and the severe pain and trauma of the procedure. Both haemorrhagic and neurogenic shock can be fatal. </p> <h3>Pain</h3> <p>The majority of mutilation procedures are performed without anaesthetics and cause the girl severe pain. Even if a local anaesthetic is used, multiple insertions of the needle are often required. </p> <h3>Urinary retention</h3> <p>Urinary Retention is very common and may last for hours or days. It is commonly due to pain, tissue swelling, inflammation, injury to the urethra, and fear of passing urine on the raw wound. </p> <h3>Injury to adjacent tissue</h3> <p>Injury to the urethra, vagina, perineum and rectum can result from the use of crude instruments, poor light, careless techniques, or from the struggles of the girl. </p> <h3>Infection</h3> <p>Infection commonly occurs for a number of reasons; unhygenic conditions, the use of unsterilized instruments, applications of traditional herbs or ashes to the wound, contamination of the wound with urine and/or faeces, or binding of the legs following infibulation which prevents wound drainage. Septicaemia and tetanus may also develop. </p> <h3>Fracture or dislocation</h3> <p>Fracture of the clavicle, femur, humerus or hip joint can occur if heavy pressure is applied to a struggling girl during the procedure - as often occurs when several adults hold her down. </p> <h3>Failure to heal</h3> <p>Wounds may fail to heal quickly because of infection, irritation from urine, underlying anaemia or malnutrition. </p> <p> </p> <h2><strong>Long term complications</strong></h2> <h3>Difficulties with micturition</h3> <p>Difficulties can occur due to damage to the urethral opening, obstruction of the urinary opening, or scarring of the meatus - and can lead to chronic incontinence or difficulty passing urine. For many infibulated girls, passing urine can take up to 20 minutes when they are still virgins. </p> <h3>Recurrent urinary tract infections (UTIS)</h3> <p>Partial occlusion of the vagina and urethra means the normal flow of urine is deflected and the perineum remains constantly wet and susceptible to bacterial growth. Retrograde UTI's therefore commonly occur, affecting the bladder, uterus and kidneys. Damage to the lower urinary tract during the procedure can also result in urinary tract infections. </p> <h3>Chronic pelvic infections</h3> <p>Partial occlusion of the vagina and urethra increases the likelihood of infection and ascending pelvic infections are common. The infections are often painful and may be accompanied by a noxious discharge spreading to the uterus, fallopian tubes and ovaries - and frequently become chronic. </p> <h3>Infertility</h3> <p>Infertility can occur due to chronic pelvic infections causing irreparable damage to the reproductive organs. </p> <h3>Vulvul abscesses</h3> <p>Vulval abscesses develop due to deep infection resulting from faulty healing or an embedded stitch causing the formation of an abscess. </p> <h3>Neurinoma</h3> <p>Neurinoma can develop when the dorsal nerve of the clitoris is cut or trapped in a stitch or in scar tissue. The surrounding area becomes hypersensitive and unbearably painful. </p> <h3>Keloid scars</h3> <p>Keloid scars result from slow and incomplete healing of the wound and the production of excess scar tissue. The scars may obstruct the vaginal opening and be so extensive that they prevent penile penetration. </p> <h3>Dermoid cysts</h3> <p>Dermoid cysts result from inclusion of the epithelium during healing, leading to swelling or pockets producing secretion. The cysts vary in size, are extremely painful and can prevent sexual intercourse. </p> <h3>Calculus formation</h3> <p>Calculus formation develop due to menstrual debris or urinary deposits in the vagina or in the space behind the bridge of the scar tissue. </p> <h3>Fistulae</h3> <p>Vesico-vaginal or recto-vaginal fistulae can form as a result of injury during circumcision, de-infibulation, re-infibulation, sexual intercourse, or obstructed labour. Urinary and faecal incontinence may be lifelong with severe social consequences. </p> <h3>Difficulties with menstruation</h3> <p>Partial or total occlusion of the vaginal opening commonly results in dysmenorrhoea or amenorrhea. Haematocolpos occasionally occurs from the retention of menstrual blood due to the almost complete coalescence of the labia. </p> <h3>Increased risk of HIV transmission</h3> <p>There is an increased risk of HIV transmission due to the use of the same unsterile instruments in-group circumcisions, repeated cutting and stitching during labour, and the higher incidence of lacerations and abrasions during intercourse. </p> <h2>Sexual complications</h2> <p>Many women who have undergone FGM experience various forms and degrees of sexual aberrations. These may include fear associated with initial sexual intercourse, pain associated with sexual intercourse, difficulty or inability to have sexual intercourse, vaginismus, and decreased sexual pleasure and fulfilment. </p> <p>It is difficult to assess the impact of FGM on women's sexual fulfilment however, as each individual woman with FGM will be affected differently. Factors such as the type of FGM and the amount of tissue removed, the extent of scarring, the experience of the initial procedure, cultural and social expectations, and affection and bonding in sexual relationships will all impact directly on sexuality and sexual functioning. <a href="http://fgm.co.nz/complications-of-fgm/#top">[back to top]</a> </p> <h2>Childbirth complications</h2> <p>There are a range of childbirth complications that can be associated with FGM, particularly with Type 3 FGM (infibulation). The extent of the complications varies depending on factors such as the type of FGM, parity, and the nature of the scar tissue. Complications that can occur following infibulation, particularly amongst primigravidas, are as follows: </p> <ul> <li>in the event of a miscarriage the foetus may be retained in the uterus or the birth canal, and performing a dilation and curettage maybe difficult</li> <li>incorrect assessment of the stage of labour, cervical dilation, and foetal presentation due to the inability to perform vaginal examinations</li> <li>inability to perform an induction with prostaglandins due to the very narrow introitus</li> <li>difficulty applying a foetal scalp electrode, performing a foetal blood sample, or inserting a urinary catheter due to the very narrow introitus</li> <li>difficulty identifying some obstetric emergencies such as cord prolapse due to an inability to perform a vaginal examination</li> <li>increased risk of bleeding, wound infection, and damage to surrounding tissues due to repeated deinfibulation, particularly if it is not performed correctly</li> <li>prolonged and obstructed labour due to partial or total occlusion of the vaginal opening. This can lead to increased risk of uterine inertia, rupture or prolapse, tearing to the perineum, haemorrhage, and fistula formation. The baby may have an increased risk of suffering neonatal brain damage or death as a result of birth asphyxia</li> <li>repetition of deinfibulation and reinfibulation weakens the scar tissue and at the beginning of menopause a woman may have a mass of fibrous tissue resulting in incontinence and prolapses of the vaginal wall.</li> </ul> <p> </p> <h2>Psychosocial complications</h2> <p>There is very limited research on the impact of FGM on psychological health. The research that has been conducted is sparse and as FGM is condoned in many of the countries where it is practiced, research is likely to have been limited by social and cultural restrictions on the exploration of any negative impacts of the practice. Some of the negative psychological effects that have been reported include feelings of anxiety, fear, bitterness and betrayal, loss of trust, suppression of feelings, feelings of incompleteness, loss of self esteem, panic disorders and difficulty with body image. </p> <p>When considering the psychosocial consequences of FGM, it is important to balance the traumatic impact of the initial FGM procedure and its long-term sequelae, against the social and cultural benefits that FGM brings to young girls in the communities where it is practiced </p> <p><br></p> <h1><strong>SAY O TO FEMALE GENITAL MUTILATION. Septicaemia and tetanus may also develop. </p>\n<h3>Fracture or dislocation</h3>\n<p>Fracture of the clavicle, femur, humerus or hip joint can occur if heavy pressure is applied to a struggling girl during the procedure - as often occurs when several adults hold her down. </p>\n<h3>Failure to heal</h3>\n<p>Wounds may fail to heal quickly because of infection, irritation from urine, underlying anaemia or malnutrition. </p>\n<p> </p>\n<h2><strong>Long term complications</strong></h2>\n<h3>Difficulties with micturition</h3>\n<p>Difficulties can occur due to damage to the urethral opening, obstruction of the urinary opening, or scarring of the meatus - and can lead to chronic incontinence or difficulty passing urine. For many infibulated girls, passing urine can take up to 20 minutes when they are still virgins. </p>\n<h3>Recurrent urinary tract infections (UTIS)</h3>\n<p>Partial occlusion of the vagina and urethra means the normal flow of urine is deflected and the perineum remains constantly wet and susceptible to bacterial growth. Retrograde UTI's therefore commonly occur, affecting the bladder, uterus and kidneys. Damage to the lower urinary tract during the procedure can also result in urinary tract infections. </p>\n<h3>Chronic pelvic infections</h3>\n<p>Partial occlusion of the vagina and urethra increases the likelihood of infection and ascending pelvic infections are common. The infections are often painful and may be accompanied by a noxious discharge spreading to the uterus, fallopian tubes and ovaries - and frequently become chronic. </p>\n<h3>Infertility</h3>\n<p>Infertility can occur due to chronic pelvic infections causing irreparable damage to the reproductive organs. </p>\n<h3>Vulvul abscesses</h3>\n<p>Vulval abscesses develop due to deep infection resulting from faulty healing or an embedded stitch causing the formation of an abscess. </p>\n<h3>Neurinoma</h3>\n<p>Neurinoma can develop when the dorsal nerve of the clitoris is cut or trapped in a stitch or in scar tissue. The surrounding area becomes hypersensitive and unbearably painful. </p>\n<h3>Keloid scars</h3>\n<p>Keloid scars result from slow and incomplete healing of the wound and the production of excess scar tissue. The scars may obstruct the vaginal opening and be so extensive that they prevent penile penetration. </p>\n<h3>Dermoid cysts</h3>\n<p>Dermoid cysts result from inclusion of the epithelium during healing, leading to swelling or pockets producing secretion. The cysts vary in size, are extremely painful and can prevent sexual intercourse. </p>\n<h3>Calculus formation</h3>\n<p>Calculus formation develop due to menstrual debris or urinary deposits in the vagina or in the space behind the bridge of the scar tissue. </p>\n<h3>Fistulae</h3>\n<p>Vesico-vaginal or recto-vaginal fistulae can form as a result of injury during circumcision, de-infibulation, re-infibulation, sexual intercourse, or obstructed labour. Urinary and faecal incontinence may be lifelong with severe social consequences. </p>\n<h3>Difficulties with menstruation</h3>\n<p>Partial or total occlusion of the vaginal opening commonly results in dysmenorrhoea or amenorrhea. Haematocolpos occasionally occurs from the retention of menstrual blood due to the almost complete coalescence of the labia. </p>\n<h3>Increased risk of HIV transmission</h3>\n<p>There is an increased risk of HIV transmission due to the use of the same unsterile instruments in-group circumcisions, repeated cutting and stitching during labour, and the higher incidence of lacerations and abrasions during intercourse. </p>\n<h2>Sexual complications</h2>\n<p>Many women who have undergone FGM experience various forms and degrees of sexual aberrations. These may include fear associated with initial sexual intercourse, pain associated with sexual intercourse, difficulty or inability to have sexual intercourse, vaginismus, and decreased sexual pleasure and fulfilment. </p>\n<p>It is difficult to assess the impact of FGM on women's sexual fulfilment however, as each individual woman with FGM will be affected differently. Factors such as the type of FGM and the amount of tissue removed, the extent of scarring, the experience of the initial procedure, cultural and social expectations, and affection and bonding in sexual relationships will all impact directly on sexuality and sexual functioning. <a href=\"http://fgm.co.nz/complications-of-fgm/#top\">[back to top]</a> </p>\n<h2>Childbirth complications</h2>\n<p>There are a range of childbirth complications that can be associated with FGM, particularly with Type 3 FGM (infibulation). The extent of the complications varies depending on factors such as the type of FGM, parity, and the nature of the scar tissue. Complications that can occur following infibulation, particularly amongst primigravidas, are as follows: </p>\n<ul>\n <li>in the event of a miscarriage the foetus may be retained in the uterus or the birth canal, and performing a dilation and curettage maybe difficult</li>\n <li>incorrect assessment of the stage of labour, cervical dilation, and foetal presentation due to the inability to perform vaginal examinations</li>\n <li>inability to perform an induction with prostaglandins due to the very narrow introitus</li>\n <li>difficulty applying a foetal scalp electrode, performing a foetal blood sample, or inserting a urinary catheter due to the very narrow introitus</li>\n <li>difficulty identifying some obstetric emergencies such as cord prolapse due to an inability to perform a vaginal examination</li>\n <li>increased risk of bleeding, wound infection, and damage to surrounding tissues due to repeated deinfibulation, particularly if it is not performed correctly</li>\n <li>prolonged and obstructed labour due to partial or total occlusion of the vaginal opening. This can lead to increased risk of uterine inertia, rupture or prolapse, tearing to the perineum, haemorrhage, and fistula formation. The baby may have an increased risk of suffering neonatal brain damage or death as a result of birth asphyxia</li>\n <li>repetition of deinfibulation and reinfibulation weakens the scar tissue and at the beginning of menopause a woman may have a mass of fibrous tissue resulting in incontinence and prolapses of the vaginal wall.</li>\n</ul>\n<p> </p>\n<h2>Psychosocial complications</h2>\n<p>There is very limited research on the impact of FGM on psychological health. The research that has been conducted is sparse and as FGM is condoned in many of the countries where it is practiced, research is likely to have been limited by social and cultural restrictions on the exploration of any negative impacts of the practice. Some of the negative psychological effects that have been reported include feelings of anxiety, fear, bitterness and betrayal, loss of trust, suppression of feelings, feelings of incompleteness, loss of self esteem, panic disorders and difficulty with body image. </p>\n<p>When considering the psychosocial consequences of FGM, it is important to balance the traumatic impact of the initial FGM procedure and its long-term sequelae, against the social and cultural benefits that FGM brings to young girls in the communities where it is practiced </p>\n<p><br></p>\n<h1><strong>SAY O TO FEMALE GENITAL MUTILATION.
