Medicare only recently began collecting and publishing payroll data on the staffing of nursing homes as required by the Affordable Care Act of 2010, rather than relying as it had before on the nursing homes’ own unverified reports. The payroll records revealed lower overall staffing levels than the homes had disclosed, particularly among registered nurses. Those are the highest-trained caregivers required to be in a nursing home, and they supervise other nurses and aides. Medicare mandates that every facility have a registered nurse working at least eight hours every day. “It’s a real positive that they actually are taking the payroll-based system seriously, that they’re using it to punish those nursing homes that either aren’t reporting staffing or those that are below the federal limit,” said David Grabowski, a professor of health care policy at Harvard Medical School. “Could they do more? Sure, but I think it’s a really good start.” The five-star rating system is a tool for families searching for nursing homes, and the rankings are published on the government’s Nursing Home Compare website. The revised ratings gave the lowest rating for staffing to 1,387 of the nation’s 15,616 skilled nursing facilities, according to a Kaiser Health News analysis of the latest data released by Medicare. They all received one star out of a possible five on July 25, when Medicare updated the ratings. Nursing home industry officials have acknowledged that some facilities are struggling to meet the new payroll reporting requirements. Katie Smith Sloan, president of LeadingAge, an association of nonprofit providers of aging services including nearly 2,000 nursing homes, said the lowered star ratings were disappointing and attributed them largely to a work force shortage. “Our members are battling on multiple fronts to recruit and retain all types of qualified staff, and nurses in particular,” she said in a statement. In footnotes on the Nursing Home Compare website, Medicare said the homes with the one-star staffing rating either lacked a registered nurse for “a high number of days” over three months, provided data the government couldn’t verify or didn’t supply their payroll data at all. The downgraded homes reported seven or more days without any registered nurses, the analysis found. For roughly half the homes, the downgrades lowered their overall star ratings, which are the measures displayed most prominently on the site. But some of the homes saw their overall ratings stay the same or even rise, buoyed by their scores on other quality measures. Seventy-nine are still rated with a coveted five stars. While the Kaiser Health News analysis found substantially lower average staffing of nurses and aides at for-profit facilities than at nonprofits and government-owned homes, the number of downgraded nursing homes was roughly proportionally divided among the three categories, indicating an industrywide issue with staffing by registered nurses in particular. The agency noted it has been preparing nursing homes since 2015 for the new payroll system. “We’ve just begun to leverage this new information to strengthen transparency and enforcement with the goals of improved patient safety and health outcomes,” the Centers for Medicare & Medicaid Services said in a statement. The payroll data, analyzed by Kaiser Health News, showed that for-profit nursing homes averaged 16 percent fewer staff than did nonprofits, even after accounting for differences in the needs of residents. The biggest difference was in the number of registered nurses: At the average nonprofit, there was one RN for every 28 residents, but at the average for-profit, there was only one RN for every 43 residents. Researchers have repeatedly found lower staffing in for-profit facilities, which make up 70 percent of the industry. The data also revealed that nursing homes have large fluctuations in staffing. The average nursing home had one licensed nurse caring for as few as 17 residents or as many as 33, depending on the day. On the best-staffed days, each certified nursing assistant or other aide cared for nine residents, but on the worst-staffed days, each aide was responsible for 16 residents. Weekend staffing was particularly sparse. On weekends on average, there were 11 percent fewer nurses providing direct care and 8 percent fewer aides. |

There have been a lot of funds stolen and people lost entire fortunes. Yes, I know, investment is risky ! But why not make the space safer ? Let's start with how the cryptocurrency space evolved lately : blockchain.jpg Blockchain implementations are error-prone : Ethereum virtual machine implementation has 703 open issues and 2186 closed issues (1/15/2018). Smart contracts are open-sourced to hackers and immutable once deployed : TheDAO - 1 bug, 50$ million lost, fixed by hard fork. Only in 2017 630 million $ have been lost to hackers in 12/2017. This year we saw the Bancor issue that led to millions of funds lost. Certik could have helped warn Bancor and prevent this. You can see how CertiK could have helped Bancor here On June 16th 2018, a bug occurred in the ICON smart contract, this error was exploited by malicious actors to affect the functionality of the network. The bug couldn't be used for stealing tokens. Yet, Certik could help ICON fix the detect ICX EnableTokenTransfer Issue. Usage The Foundation behind the project has developed a one-stop solution, called the CertiK Platform, which provides a powerful and efficient set of Certified Kits for building fully trustworthy blockchain ecosystems. The Platform is comprised of: Smart labelling : The CertiK Platform has designed a labeling approach in order to specify dApps/systems. These labels are expressive enough to formally state the desired properties and are compatible with existing programming languages such as Solidity. Also, via the utilization of deep learning techniques with a manually established labelled code base for training, the CertiK Platform intends to introduce a smart labelling framework, to help understand decentralized programs at both the syntax and semantics levels, and then automatically add proper labels to the source code. Layer-based decomposition : The CertiK team is striving to achieve modular verification by realizing a named layered deep specification concept. This technique uncovers the insights of layered design patterns and makes it possible to break down a complex proof task into smaller ones, and verify each of them at their proper abstraction level. Pluggable proof engine: The smaller proof obligations are easier to untangle and can be solved by some automatic verifiers such as SMT solvers. To enable extensibility, the CertiK Platform is intended to provide an open protocol whereby significantly more advanced solving algorithms can be freely plugged into this system. Machine-checkable proof objects : The CertiK Platform constructs mechanized proof objects (or counterexamples) in such a way that these proofs can be quickly checked by anyone using their own machine. These proof objects can be viewed as the “certificates” to the verified programs. Certified dApp libraries : The CertiK Platform also offers a series of certified libraries and plug-ins to the integrated development environment (IDE) for building more trustworthy dApps. These libraries help improve the code quality and reliability of the entire blockchain community, and the use of these tools will cost a small amount of CTK as virtual crypto “fuel”. Customized certification services : For dApps/systems with high reliability requirements, such as digital wallets, the CertiK Platform intends to provide customized certification services. In such cases, verification experts will help specify/verify the programs and generate a detailed, comprehensive report. usage.png CertiK system mainly involves 5 parties with each individual being assigned different tasks and roles to complete the system. Customers : will be code developers or automatic programs that send codes to the system. The sent codes are called “Proof objects” and must be sent along with CertiK tokens (CTK) as a reward. Bounty hunters : their tasks are to allocate computing resources to the system as well as to receive proof objects and to broadcast the works within the system. They are entitled to be rewarded with CTK tokens sent by the customers. To be qualified in this role, a certain party must possess a high number of CTK tokens as required by the system. Checkers : their responsibilities are to record transactions or verify proof objects submitted by bounty hunters. Bounty hunters will receive the tokens when the checkers confirm the proof objects. Upon confirmation, the checkers will be rewarded with some CTKs. Sages : will be connectors of proof engine to CertiK system. The proof engine is an algorithm deployed to find bugs in smart contracts. Sages will be rewarded based on performance of their engines. The bounty hunters will be users of proof engines Users : this is suitable for developers who can link library of CertiK to IDE in order to create DApp. Users must pay expenses in CTKs. Users can subscribe to all CertiK Platform’s certified libraries and IDE plug-ins to build their own DApps/systems with some CTKs These five roles will help to maintain the stability and sustainability of the CertiK ecosystem and can also lead to a natural growth in the importance and value of the CTK token. CertiK makes use of its native CTK token which is used as a digital fuel across the platform. CTK is a non-refundable functional utility ‘fuel’ which will be used as the unit of exchange between participants on the CertiK Platform. CTK is also used in economic incentives which encourage participants to both contribute and maintain the ecosystem on the CertiK Platform. Competition and Development Roadmap CertiK competitors are Quantstamp and Zeppelin. Quantstamp system is based on manual checking. Altough Quantstamp has also planned to have automatic checking, according to CertiK's roadmap, they will be the first to launch the full system. Quantstamp's traditional system requires a large ammount of human effort in order to review the source code and manually write the specification. Zeppelin uses the same manual checking system. Another negative aspect is that the libraries provided by them are either not verified or do not offer mechanized proof of objects. Conclusion CertiK is a very interesting project because blockchain security is still a growing major problem. Smart contracts are very vulnerable and can create huge problems if the code isn't good. Investors need a safe environment in order to be able to trust projects with their money. Good security can also make institutional investors more likely to support the cryptocurrency space. There's not to much competition in this field. If CertiK is able to deliver according to their roadmap, the project will certainly be a success. Taking in consideration their partnerships, I think that's inevitable. CertiK is a real game changer and it's one of my top 3 upcoming favorite projects. Quantstamp's traditional system requires a large ammount of human effort in order to review the source code and manually write the specification.\n\nZeppelin uses the same manual checking system. Conclusion\nCertiK is a very interesting project because blockchain security is still a growing major problem. Smart contracts are very vulnerable and can create huge problems if the code isn't good. If CertiK is able to deliver according to their roadmap, the project will certainly be a success. | body |  I'm going to try to help you, contributors of the development section, to get better rewards for your dedicaded work, your projects. Please note that all I'm going to say is my point of view and other moderators may not agree with me. I'll try to be as general as possible although I prefer reviewing javascript (Node and React) contributions so the examples I'll provide will mostly be for javascript. I sometimes review .NET and Java but not so much. Utopian Guidelines First let's take a look at the utopian guidelines  Submissions should include a comprehensible commit history. Projects or updates submitted in a single commit will not be considered for potential reward. Here the easiest way for us moderators (well for me at least) to review your code is if you use Pull Requests. 1 feature = 1 pull request = multiple organized commits. We can quickly see all the modifications that are in the contribution. It's also the way I'm used to develop. I was a mentor for a while at OpenClassrooms and part of the course was to learn how to use a Pull Request. The use of PRs was mandatory in all the courses projects. Inclusion of outdated or low quality code may lead to a lower score of your contribution, thus reducing your chances of receiving a reward. Here this is quite simple, try to have updated packages and follow best practices of the language of your choosing. Generated code or other results of automated processes will not be considered for potential reward. Try to separate the generated code from your PR, commits and contribution. For example if you're using vue-cli create a Pull Request, init your project, commit and merge. But don't add this Pull Request to the contribution. It looks like you're trying to hide your real work and try to fish for a high reward by saying 'Oh look at all the code I have done'. This puts me in a bad mood and I don't want to be nice anymore. Submitted contributions should offer unique value. Redundant projects, trivial code snippets, example code or simple templates will not be considered for potential reward. Don't create contribution that just fix bugs. They must provide nice additions, features... Don't reinvent the wheel too, if a project exist in blue don't create the same one in red ;) Projects can also not concern the Steem Blockchain. To be considered for potential reward, Bug Fixes and New Features should be submitted via Pull Requests. The Pull Request should have been merged within the past 14 days. Bug fixes are OK only if it's because of a 3rd party libraries. And correcting 2 bugs isn't enough for a contribution. You'll just end up with a 0. Updates on Own Projects may be committed directly, without a Pull Request. To be considered for potential reward, commits should not be older than 14 days. That's one of a guideline that I don't specially agree with. I'd rather have pull requests and it's a really good habit to have especially when you work in a team. Why code differently because you're a team of one? Bug Fixes for contributor’s Own Projects will not be considered for potential reward, unless the Bugs were caused by third party dependencies. We talked about this one already. The questionnaire The source of absolute truth! This is the tool that will decide the score of your contribution. For this one I like to have pictures, code extracts, justification of technical choices. For each new feature it's nice to have a little gif demo when it's possible. It's also nice to link the first post that present your project because it's not always the same moderator that will do the review. Keep in mind that you're presenting your work of your open source project to the community not just to an utopian moderator so you can get a reward. So each contribution should be presented as an article that's pleasant to read. Not just 'Hey I've done some code what is my utopian score?' . Read this contribution. It's a good example of how to achieve the highest score for this question. How would you rate the impact and significance of the contribution to the project and/or open source ecosystem in terms of uniqueness, usefulness and potential future applications? This one is highly subjective. If it's a few fixes, cosmetic addons I won't give a high score. But if there are multiple new core features added to the project I'll give the highest score. How would you rate the total volume of work invested into this contribution? Your project is for the open source community, it's not a daily paid job. So in my opinion, I won't give a high score for something that could be done in a few hours. But how do I judge the amount of hours? Well I can only do it based on my experience. I've been a developer for 11 years now. I've worked with multiple languages (.NET, Java, PHP, JavaScript, ...) for multiple companies. So I guess the amount of work based on how much time it would take me and then add a little more hours because it wouldn't be fair to junior devs. So if it's one day it's going to be "shows some work done", if it's 3 or more I give the highest score. How would you rate the quality of the code submitted? Well now we have best practices and IDEs that can automate a few tasks for us. So I'm looking for design patterns like MVC, MVVM, ... I'm also looking to the overall aspect of the code. IDEs can now automatically format code when saving. We also have tools to check our code or modify it. I'm thinking about eslint and prettier for example. IDE can also suggest changes. If you're used to write java 1.4, you install java 1.8, IntelliJ will offer tips on how to write code with the new features (streams, ...). The last part are the algorithms, the developer work and here this is more the experience talking. Don't hesitate to take a look at well known, updated projects. There so many ways to write an algorithm and there isn't always a best way to do it. How would you rate the proficiency and expertise necessary to fix the bug / implement the added feature(s)? The more complicated a project is the more expertise it requires but if a contribution only concerns adding a lot of HTML templates well you don't need to be an expert to produce code of good quality. Let's take a JavaScript example. If you deal with ReactJS, Redux, RxjS and a database like PostgreSQL or MongoDB and then you add docker, etc. This begins to require a lot of basic knowledge on multiple tech. Or for Java if you have SpringMVC, Spring Security, Maven, Hibernate, you add a little AngularJS and unit tests with JUnit, again this is a wide range of stuff to know. So if your contribution uses a bit of everything then you'll get the high score! How would you rate the accuracy and readability of the commit messages? Here it's pretty easy to have a high score. Avoid commit messages like "minor fix, add CSS, add stuffs, oups, adding a lot of features". Instead don't hesitate to split your commits into meaningful ones. Let's say you're adding a whole CRUD feature, your commits could be divided like that: "Add X packages", "Add API CRUD endpoints", "Add unit tests", "Fix bug for the issue X", "Add CRUD pages", "Add jest tests or E2E tests", "Fix CSS issue for XXX", "Add responsive to the CRUD pages" And there you have it. The commit history helps us understand how the developer did his feature. We know what code to expect in each commit. So top score! Here I have a great example: this contribution and this pull request. You can see the clear commit history. How do you rate the quality of the comments in the code? This last question has raised a lot of issues. I've see a lot a useless comments. The aim of a comment is to help to quickly understand a portion of code. So when I see something like this I get angry. Some comments that I found not useful (sorry guys). Example 1, example 2 // Get the user profile const profile = await Profile.findOne({ userId }); You should comment the function declaration for example. Here is a random example. /* Process the invoice received. If the invoice is paid ship the product otherwise send an email to the user and back-office @param user application client @param invoice product invoice */ function processInvoice(user, invoice) { } or part of code that is complicated // Explain the content of what it's done because if someone new (or the 6 month later you) read it, it will take a few minutes to understand if (somevar === somestuff) { complicated code } And what if I don't comment anything because my project is easy? Well you'll probably have the lower score but that doesn't mean you won't be picked by the staff and have the maximum reward. In short Take your time to write your article, be a salesman and offer everyone (not only the utopian moderators) something nice to read. Don't post multiple little contributions. The reward increases exponentially so one well written contribution will have a greater score than 3 average ones. Try it be up to date in the languages you've chosen, read articles on best practices, design patterns, ... Simplify your job and try to automate some tasks (code format, ...) I hope this little article will help get better rewards and I hope to see exceptional contributions in the future. May be we will have to fight between moderators for the staff picked contribution. Comcast originally said it was raise the offer on the table after Disney topped it but debt obviously became a consideration. Already one of the most indebted companies in the world, taking on this acquisition would have furthered that path.  Another consideration might be the DOJ's recent contesting of the ATT-Time Warner deal. It is possible that Comcast would have faced similar resistence if it went ahead with the buyout of Fox. The deal with Disney-Fox could face a similar situation from the EU meaning selling off the ownership is Sky could be mandated. Some believe this is a non-issue for the Disney team since this deal benefits them greatly without the Sky network.  Wall Street already anticipated this move. It did not believe Comcast would beat out Disney who was the preferred choice by Rupert Murdoch himself. Both stocks traded higher on the news.
