ADHD merupakan gangguan jangka panjang yang menyerang jutaan anak dengan gejala-gejala yang dapat berlangsung hingga dewasa. Siapa saja memiliki kemungkinan untuk menderita ADHD, tapi kondisi ini umumnya dialami oleh orang-orang dengan gangguan belajar.\n\nBeberapa gejala dalam perilaku yang dialami penderita ADHD meliputi sulit konsentrasi serta munculnya perilaku hiperaktif dan impulsif. Gejala-gejala ADHD umumnya terlihat sejak usia dini dan cenderung makin jelas ketika terjadi perubahan pada situasi di sekitar sang anak, misalnya mulai belajar di sekolah. Sebagian besar kasus ADHD terdeteksi pada usia 6-12 tahun. Anak-anak dengan ADHD cenderung rendah diri, sulit berteman, serta memiliki prestasi yang kurang memadai.\n\nADHD cenderung lebih sering terjadi dan mudah terdeteksi pada laki-laki daripada perempuan. Contohnya anak laki-laki umumnya memiliki perilaku yang lebih hiperaktif sementara anak perempuan cenderung lebih diam, tapi sulit berkonsentrasi.\n\nFaktor Risiko dalam ADHD\nPenyebab ADHD belum diketahui dengan pasti. Tetapi sejumlah penelitian menunjukkan bahwa ada beberapa faktor yang dapat memengaruhi tingkat risiko seseorang.       "body": "www.wired.com\nAdam Rogers\n\nThe viral disease Ebola has, as of May 26, killed 25 people in the Democratic Republic of Congo and sickened 31 more. In response, treatment centers have popped up (two of three people who fled one of those centers in the city of Mbandaka have died) and health care workers there are getting a still-experimental vaccine. People who’ve had contact with someone with Ebola, and their contacts, will get the shot, too. Heroic health workers are caring for the sick and doing epidemiological detective work. And the key to stopping the outbreak before it unfolds into a nightmare might be high-tech synthetic ice.\n\nOn that note of wait-what, let's back up for a second. At the tail end of the Ebola epidemic in West Africa between 2013 and 2016 that killed 11,000 people, a global network of researchers took an Ebola vaccine off the shelf where it had been sitting, metaphorically, since its development in the aftermath of the September 11 attacks. They put it into serious field testing, and those studies turned into a good-enough confirmation of the effectiveness of the vaccine, rVSV-Zebov, that health agencies are deploying it in Congo right now.\n\nOut of the dozen or so possible Ebola vaccines, rVSV-Zebov has a lot going for it. The VSV there is vesicular stomatitis virus, which doesn’t infect humans, and here scientists have replaced a particular glycoprotein with one from the Zaire strain of Ebolavirus (the “Zebov”). It’s a so-called live attenuated vaccine, a potent inducer of immunity that requires just one dose.\n\nGood stuff; in 2016, GAVI, the Vaccine Alliance, set up approvals to have 300,000 doses ready to go in case of another outbreak just like the one going on right now. Just one problem: “This vaccine, if I have to be absolutely honest with you, they started out working with the vaccine quite cold, and then they looked at taking it up to a normal temperature,” says Seth Berkley, an epidemiologist and CEO of GAVI. “They haven’t done all of the stability and process work they might do.” In other words, the developers of the vaccine didn't have time to figure out how to formulate it to be stable at the usual 2 to 8 degrees C. (That's 35.6 to 46.4 degrees if you're Team Fahrenheit.)\n\nPoint is, all signs indicate the vaccine works. But until you’re ready to use it, you have to keep it waaaaay colder, frozen at -60 to -80 degrees centigrade. (yes, fine, -76 to -112 degrees F, you barbarians). So that presents a particularly irksome version of a problem familiar to vaccine makers: the cold chain.\n\nIn parts of the world with limited electricity, even the cold-but-above-freezing temperatures are a challenge. To meet it, the Bill Gates-funded Global Good,1 science-hero arm of ex-Microsoft CTO Nathan Myhrvold’s Intellectual Ventures, built a solution. Their answer came to be called the Arktek, possibly the coolest cooler on Earth.\n\nBased loosely on the dewars that industrial applications use to contain things like supercold liquid nitrogen, the Arktek is a double-walled vessel with near-vacuum in between, held together with reinforced fiberglass. “You’re trying to reduce all the mechanisms of heat transfer between the warm outside and the cold inside,” says Mike Friend, principal investigator at Intellectual Ventures. The vacuum reduces convection. A shiny coating on the inner vessel cuts down thermal radiation. The low conduction of the reinforced fiberglass keeps heat from leaking in that way. “And then you need a thermal battery, so to speak.”\n\nIn this case, that battery supplies cold, not heat. In the original Arktek, it was simple water ice, frozen in semicircular plastic containers and then allowed to warm to slightly above freezing. Eight of them placed in the Arktek would keep the thing at the right temperature for up to a month, even in the most sweltering equatorial heat. So once a month, villages could get their vaccines and ice blocks resupplied from somewhere with freezers, and meanwhile everyone could get vaccinated. Intellectual Ventures developed the tech and licensed it, for free, to the Chinese refrigeration company Aucma to build the coolers.\n\nWas it a perfect system? Well, given the opportunity to have light refrigeration, some folks took slight advantage; one solar refrigerator technician reported an Arktek smelling suspiciously of fish upon inspection, and anything that’ll keep vaccines cold will do the same for beer. But still.\n\nEbola upped the ante. To get down to below -60 F as recommended by Merck, the manufacturer of the vaccine, ordinarily you’d use dry ice—frozen carbon dioxide. It’s the right temperature, and it sublimates into a gas instead of melting into a liquid. That requires more energy, which means dry ice is a very good way to store cold. “But for Sierra Leone, Guinea, and Liberia, the issue was they could not get a reliable supply of dry ice, and power was a big issue, too,” Friend says. “The CDC and WHO wound up being able to only set up warehouses with reliable power in the capital, and they had to go through extreme measures to do that.”\n\nThat meant a whole new logistical effort. Local health workers and international aid groups retrofitted warehouses with power supplies, alarms and switches, sub-80 degree freezers, air conditioners, and so on. Because the “ring vaccination” strategy scientists wanted to test in West Africa meant that they’d have to go out to where Ebola cases were and then track down contacts and contacts of contacts to vaccinate. It could take days, and the vaccine had to stay Arcticly frosty for most of that time.\n\nWHO called Friend’s team. It was November of 2014; they had until January to find a solution.\n\nThis is where the synthetic alcohol ice comes in. Water ice wasn’t cold enough. Dry ice wasn’t available. Friend needed a different thermal battery.\n\nHe went looking in a category of chemicals called phase-change materials, PCMs, synthetic molecules built with specific freeze-thaw cycles. And he found one: a proprietary PCM based on a denatured alcohol, made by a small company in England. “The owner is a very reserved man, I think Eastern European. Trying to get information out of him was a bit challenging,” Friend says. Like, for example, why exactly was he making this stuff? “It was a special request. He didn’t give very many details about who had requested it.”\n\nIt didn’t really matter; he was willing to make it for Friend in just a couple of weeks. “Then we had to validate that it actually worked, actually simulating what would be done in the field,” Friend says. They loaded the PCM into the plastic semicircular bricks, froze them, and loaded up Arkteks kept in rooms heated to West African extremes. Then they opened and closed them just like field workers would, monitoring the internal temperature. The PCM was flammable; it would have to fly to West Africa on a military cargo plane. Friend's team wasn't even sure that the Arktek itself would hold up at the lower temperature, much less the plastic trays used to hold the vaccine. “We had to bring them down to temperature, vibrate them, drop them, see how they would perform,” Friend says. “Luckily everything was OK.”\n\nNot quite everything. The high-density polyethylene used to make the plastic bricks didn’t agree with the PCM; after a few freeze-thaw cycles, the PCM started to leak out through the screw-down top. They could smell it. But Friend’s team had made aluminum containers, just in case.\n\nThe aluminum containers worked. Modified Arkteks are at the core of the logistical strategy for full-on ring vaccination in Congo. WHO is using two freezers capable of getting below -86 degrees C in Kinshasa, the country’s capital, to store vaccine and freeze the PCM ice packs, along with a power source and a generator. Mbandaka, a city of perhaps 1.3 million where four cases have emerged, has two more freezers, a 300-liter one for ice packs and a 600-liter one for vaccine. And they’re using 16 Arkteks in the field. They can keep vaccine below -65 C for five days, and experience has shown that it’ll still work after sitting between 2 and 8 degrees for up to two weeks. (Friend has since slightly modified the PCM so it’s less flammable.)\n\n“People initially said, oh my god, this is going to be impossible,” Berkley says of the cold chain. “It’s not ideal, but it’s not going to be the problem.”\n\nIn fact, with the vaccine safe, the bigger concern is going to be making sure people feel OK taking it. “Everybody knows what a vaccine is, but it’s usually either given as part of a campaign or part of routine immunization,” Berkley says. “The concept that you go into a community, visit the family of someone who’s sick, and you vaccinate just that family and the people around them is something that has to be socialized.” People who work in the area, who speak the language, who are from DRC will be the ones who have to deliver that message and get informed consent from vaccinees. But if the program works, it could mean people won’t be as fearful of treatment centers and of talking to epidemiological workers trying to figure out the rings of people who need to be vaccinated.\n\nWhether ring vaccination will work fast enough remains to be seen. Researchers are going to be taking data on this deployment of rVSV-Zebov, as well as looking at other even more experimental therapies—the antibody drug ZMapp and the antivirals favipiravir and GS-5734. According to Nature, the Congolese Ministry of Health and an ethics board have yet to approve those other drugs. “Deploying something that came together as the result of all these people was very gratifying,” Friend says. “The fact that the vaccine was effective and we were able to get it out there, that’s amazing.” Good epidemiology, treatment, and a vaccine (thanks to that neo-ice) are still Congo’s best hope.\n\n1 UPDATE 5/29/18 5:18 PM Corrected the source of Global Good's funding\n\nMore Great WIRED Stories",

However, this tendency may also underly the seeming contradiction that large corporations, which typically exert centralized control over a product or service, are embracing new decentralized technologies such as blockchain and the token economy.\n\nKakao is a corporate giant that dominates the Korean mobile market. Its mobile messaging app Kakaotalk accounts for 94 percent of the domestic market. Based on this overwhelming market share, Kakao has successfully expanded its operations into markets such as advertising, gaming, mobile banking, taxi services and music.\n\nAs a result, Kakao employed 2,600 staff as of 2017 and recorded annual sales of 1.9723 trillion Korean won ($1.8 billion) with an operating profit of 165.3 billion KRW ($154 million).\n\nThe company recently announced a new blockchain business.\n\nAs one of the leading \"middlemen\" that has taken control of the mobile platform market in Korea, if Kakao chooses to embrace blockchain – which eliminates the middleman and allows users to directly exchange value – what would happen to the massive profits the company currently earns in its intermediary role?\n\nThe 'middleman' exists for a reason\nSeeking the answer to that question and more, CoinDesk spoke to Jason Han, the CEO of Ground X – a Kakao subsidiary that has been charged with taking forward the firm's blockchain strategy.\n\n\"Will the middleman really disappear? Are the middlemen all bad? I don't think people are thinking seriously about these questions,\" he said.\n\nHan is also skeptical about whether the numerous ICO projects currently underway will be able to produce services on the same level as companies like Kakao or Amazon.\n\n\"ICOs have an odd structure. You've already made all of your money by the time you start. Let's say you can't dispose of the money for two years. Two years go by very quickly. You spend those two years working on development to get your smart contracts working, then release your service. Maybe 1,000 users sign up. But you need something more to attract more users beyond that. You have to spend a lot on marketing and go through all kinds of trouble to get your name out there. But why bother doing all that? You've already made all your money,\" he said.\n\nBefore joining Kakao in March, Han worked as the CTO of an investment company called FuturePlay, where he was responsible for evaluating the technology of companies that were potential investment targets.\n\n\"From my investment experience, there is a huge gap between what startups promise in their white papers and what they actually deliver,\" he said.\n\nThat gap cannot be bridged simply by developing good smart contracts or designing a good token economy, argued Han.\n\n\"Those who have real experience in business will know. When you finish development, it's not the end of the project – it's the beginning. Figuring out how to close that gap is a business problem,\" he continued, adding:\n\n\"Until last year, nobody talked about business when they were making dapps (decentralized applications), but I think things will be different this year. Decentralization can't be the core of a service, it's just a method of providing the service. Tokens can't be the only kind of value that you offer to users. Tokens are only part of the value. What's important is the essence of the business itself.\"\n\nPinpointing the benefits\nKakao's proposed blockchain platform, Han said, could be summed up as \"partial or gradual decentralization.\"\n\n\"Everyone is using blockchain these days because it's popular,\" he said. \"I wish people would stop trying to put everything on blockchain.\"\n\nSome of Kakao's services could be decentralized, while others cannot, Han explained. Rather than uploading a whole service to blockchain, certain functions of the components that make up the service could be separated out for decentralization and tokenization.\n\n\"For example, let's say that we convert the mileage points system into tokens. From the company's perspective, it's just changing the money into a token format. If we did that, it would be more about making mileage more useful rather than sharing our profits with users. When you tokenize non-core aspects of the service, it allows the company to retain its profitability model while also expanding user inflow. With a forward-looking approach, tokens could also be used to provide incentives to users, but in that case there need to be benefits for Kakao as well,\" Han said.\n\nSo, what are the benefits Kakao is seeking through blockchain?\n\n\"The token economy is a business model that no one could have imagined before,\" he said. \"Until now, Kakao has only operated in Korea, but through blockchain we could expand into the global market. That means taking a portion of the profits we earn as an intermediary and using it to expand our market by sharing it with users.\"\n\nFor example, he said, if Kakao were currently earning 100 units in profits, then growing the market to 1,000 units would eventually allow the firm to give 300 back to users and keep 700 for itself.\n\n\"What kind of strategy could Kakao adopt if it wanted to export its successful service to the Asian market? Each country already has one or two established services. The only solution I can think of is offering incentives through blockchain. With blockchain, you can shake up the existing order.\"\n\nCompetitors on both sides\nKakao's longstanding rival Naver is also jumping into the blockchain industry with its subsidiary, LINE Plus.\n\nNorth America-based messenger service Kik is also working on a blockchain business called KIN, while Telegram has raised $1.7 billion through an ICO, the largest amount ever recorded in a token sale.\n\n\"The biggest concern for dapps on ethereum or EOS is how to attract users,\" Han said. \"Messenger services like Telegram have an advantage since they already have a platform with an established user base. This is why we need to pay attention to what these companies are doing. On the other hand, those messaging services have lots of users but no other services, while Kakao already provides a wide range of services.\"\n\nAlthough Han was skeptical of ICO projects, he does not believe that all of these startups are destined to fail.\n\n\"Twenty years ago, nobody could have imagined a company like Google or Amazon,\" he said. \"There are so many teams working hard on their own projects, and some of them will be able to create new types of commerce that are completely different from anything we currently know. Some of them may be able to create innovative business models by focusing on decentralization. With blockchain, I think this will only take five or 10 years, not 20.\"\n\nHan provided the advertising market as an example to explain the competition he expects to see in the token economy of the future. In that industry, there are already many blockchain projects seeking to connect advertisers and consumers, thereby generating economic benefits for consumers.\n\nAt first, it will be difficult to completely remove the middleman and make the service 100 percent decentralized. Since it takes money and effort to attract advertisers and consumers, at first the middleman might take 70 percent of advertising revenue, while consumers would receive the remaining 30 percent. This model would already be competitive against current advertising agencies, where the middleman doesn't pay a cent to consumers.\n\nCompetition may lead some companies to offer 50 percent to consumers, then 70 percent, then 100 percent, eventually creating a completely decentralized system.\n\n\"Right now, nobody knows what kinds of services might come out of the blockchain world,\" Han says. \"We haven't seen them yet. Steemit has shown us some of the potential, but nobody knows how far it will grow. But if you sit on your hands for a while and then something new comes out, it will already be too late. In this situation, having a go is the right thing to do.\"\n\nTechnical issues and ICOs\nLINE Plus recently announced that it plans to create a blockchain platform using the ICON network. Kakao also plans to use an existing blockchain platform. Since most platforms released so far are open source, Kakao will use the source code of a suitable existing platform and add its own unique functions on top.\n\nHan explained that Kakao was considering many existing platforms including ethereum, tendermint, cosmos, quorum, EOS and Hyperledger.\n\n\"Ethereum has the highest level of completion,\" he said. \"But if we want to get started quickly then quorum is also a good option. EOS has not yet released its main network, so it is difficult to conduct a deep analysis of the platform.\"\n\nHe added that most existing projects have problems with performance or processing speed, and there are a host of issues outside of that which also need be resolved. The most important factor is providing a user experience that meets the expectations of the average users.\n\n\"If a platform has a very well-refined philosophy, will users be willing to pay for that value? You need to be issued a private key and install a wallet, but if you lose your private key, you lose all your money. Even the person who created the platform can't recover the money for you. It's ridiculous to the average users. Performance and speed are the basics, but even once you've got that sorted, it's just the beginning.
