| title | Personal Characteristics of a Psychotherapist |
| body |

Photo by Cathal Mac an Bheatha on Unsplash

A few days back I wrote a poem about characteristics required to be a good therapist and to practice therapy authentically. So, I thought of sharing it here. Would love to hear your feedback on it!

As I sit silently With the exit of last client of the day finally. It has been 45 years And still with each client I discover new spheres.
Being a wounded healer Personal therapy keeps checking on my countertransference. I encourage clients to examine their own feelings, thoughts and actions Rather than attempting to impose my values during the interaction.
In strive to be an efficient therapist I have evolved to be an ablest. And while maintaining qualities of a therapist Acknowledging my real self is a must.
After interacting with so many clients Over time there are many things I have learnt. More than focusing on technique Achieving therapeutic alliance is what I seek.
I have developed a strong identity Which allows me to being open to change and diversity. And attaining interpersonal expertise Helps me in maintaining healthy boundaries.
Appreciating and respecting my self Enables authenticity, sincerity and honesty itself. My decisions allow me to live my life with cheer And along comes some good sense of humor with some tears.
Doesn’t mean I am perfect, I also make errors from the misconception But I don’t dismiss them rather learn and move on. Not in the past or future but living in the present Inspires me to think of other’s contentment.
Being mindful of mine and other’s culture and differences Allows me to get involved deeply to derive meaning of their experiences. I am passionate and courageous To support those who feel helpless.
I know, these are too many good qualities to be a human But rather than being black-or-white I sway on the continuum. These are few qualities that I try to bear While sitting in a therapist’s chair. |

| title | Memory Disorders, Assessments and Rehabilitation |
| body |
**Common Memory Disorders** Memory difficulties are often the first and the most prominent sign of an acute or insidious cerebral dysfunction. Some of the common disorders of memory are- Amnesia, Dementia, Parkinson’s disease, Stroke, Traumatic brain injury, Alzheimer’s disease, Huntington’s disease, Agnosia, Korsakoff’s syndrome, and many others. In the diagnosis of these disorders, it is helpful if the assessments are used. Assessments make the diagnosis accurate. **Assessments** Most of the neuropsychological assessments include the tests of memory and the clinician will select the tests, which will answer the questions that differ according to the context of the referral. All the studies were through descriptions and little attention was given to biological aspect of the brain. But, now the brain process and cognition is studied together to understand the memory better. The purpose of the assessment is to determine the diagnosis and the performance in the test will help to determine the severity of the functioning and maybe the cause of the impairment. Repeat assessments are required when monitoring the recovery or the decline of memory function. The assessment will help us to target the problem and its severity, which will help us to identify weakness of the individual and hence in planning the rehabilitation program. Before doing the assessment it is very important that the detailed case history of the client is taken. For memory evaluation, during the clinical interview, it is very important to note about the memory symptoms, which patient/ informant describes during in-take; and an indication of patient’s memory functioning gained from the general information. **Rehabilitation** People with brain-related problems face memory and learning difficulties, although it is possible to reduce the everyday problems, which these people face. Recovery is regarded as resumption of normal life even though there maybe minor neurological and psychological deficits. There are few factors, which affect the recovery, namely, age, cognitive reserve, gender, etc. Sometimes our brain recovers naturally through the process of regeneration or plasticity. It cannot be said that how much recovery will take place. It is possible that memory shows less natural recovery over time than other functions such as language. For memory, compensation worked better. There are different ways through which memory can be improved: ***Compensatory approaches***- The method of choice of reducing everyday problems is to teach the patient the compensatory approaches. Several studies show that people with memory impairments can function independently if they are able to use strategies to get around their difficulties. There are evidence to suggest that rehabilitation can improve cognitive functioning. There was a woman who became amnestic due to neurological problem and she was taught to use a personal organizer (datebook). So, she could write and refer to the book regularly and she could easily monitor her daily events. Over a period of time, she was able to learn the different sections of the book and used to manage her life successfully. After leaving the center, she started working and was then taken on as a paid employee. Compensatory technique can assist independent living. One other instrument, which can help in number of memory impairments, is NeuroPage. This is an alphanumeric pager, which sends out daily reminders. Studies show that NeuroPage has been a great success in helping the patients to remind events. ***Errorless Learning***- It is a learning condition, which involves elimination of errors during the process of learning. Techniques of improving learning in memory-impaired people are a major part of rehabilitation. The elimination of errors can be achieved by variety of means, when we are talking about memory rehabilitation, and these comprises of (i) breaking the task in small and discrete steps; (ii) by providing sufficient models before asking the patient to perform the task; (iii) by encouraging the patient to avoid guessing; (iv) immediately correcting the errors of patient; (v) by reducing the prompts. It has been studied that people with amnesia learn better if they are prevented from making errors during learning. It is better to avoid the incorrect answer, but if failed then it is important to correct them immediately and not reinforce the response. Though many answers are yet to be answered, it is better to use errorless learning with the clients rather than using the trial-and-error learning, to help them cope in everyday life. ***A Holistic Approach-*** Holistic program addresses the emotional problems along with the cognitive deficits. Many times, the individual with cognitive deficits face psychological problem, like depression. Such programs feature both individual and group therapy, embedded with goal planning approach to reduce emotional and cognitive problems. Long-term goals are those, which should be achieved by the time of elimination of rehabilitation process, whereas short-term goals are the steps taken to reach to long-term goals. Each long-term goal would require a short-term goal. It is necessary to address cognitive and emotional difficulties. It is required to involve the patient and the family in the process of decision-making. "title": "Memory Disorders, Assessments and Rehabilitation",
"body": "\n\n
\n\n**Common Memory Disorders**\n\nMemory difficulties are often the first and the most prominent sign of an acute or insidious cerebral dysfunction. Some of the common disorders of memory are- Amnesia, Dementia, Parkinson’s disease, Stroke, Traumatic brain injury, Alzheimer’s disease, Huntington’s disease, Agnosia, Korsakoff’s syndrome, and many others.\n\nIn the diagnosis of these disorders, it is helpful if the assessments are used. Assessments make the diagnosis accurate.\n\n**Assessments**\n\nMost of the neuropsychological assessments include the tests of memory and the clinician will select the tests, which will answer the questions that differ according to the context of the referral. All the studies were through descriptions and little attention was given to biological aspect of the brain. But, now the brain process and cognition is studied together to understand the memory better.\n\nThe purpose of the assessment is to determine the diagnosis and the performance in the test will help to determine the severity of the functioning and maybe the cause of the impairment. Repeat assessments are required when monitoring the recovery or the decline of memory function. The assessment will help us to target the problem and its severity, which will help us to identify weakness of the individual and hence in planning the rehabilitation program.\n\nBefore doing the assessment it is very important that the detailed case history of the client is taken. For memory evaluation, during the clinical interview, it is very important to note about the memory symptoms, which patient/ informant describes during in-take; and an indication of patient’s memory functioning gained from the general information.\n\n**Rehabilitation**\n\nPeople with brain-related problems face memory and learning difficulties, although it is possible to reduce the everyday problems, which these people face. It is possible that memory shows less natural recovery over time than other functions such as language.\n\nFor memory, compensation worked better. There are different ways through which memory can be improved:\n\n***Compensatory approaches***- The method of choice of reducing everyday problems is to teach the patient the compensatory approaches. Several studies show that people with memory impairments can function independently if they are able to use strategies to get around their difficulties. There are evidence to suggest that rehabilitation can improve cognitive functioning.\n\nThere was a woman who became amnestic due to neurological problem and she was taught to use a personal organizer (datebook). So, she could write and refer to the book regularly and she could easily monitor her daily events. Over a period of time, she was able to learn the different sections of the book and used to manage her life successfully. After leaving the center, she started working and was then taken on as a paid employee.\n\nCompensatory technique can assist independent living. One other instrument, which can help in number of memory impairments, is NeuroPage. This is an alphanumeric pager, which sends out daily reminders. Studies show that NeuroPage has been a great success in helping the patients to remind events.\n\n***Errorless Learning***- It is a learning condition, which involves elimination of errors during the process of learning. Techniques of improving learning in memory-impaired people are a major part of rehabilitation.\n\nThe elimination of errors can be achieved by variety of means, when we are talking about memory rehabilitation, and these comprises of (i) breaking the task in small and discrete steps; (ii) by providing sufficient models before asking the patient to perform the task; (iii) by encouraging the patient to avoid guessing; (iv) immediately correcting the errors of patient; (v) by reducing the prompts.\n\nIt has been studied that people with amnesia learn better if they are prevented from making errors during learning. It is better to avoid the incorrect answer, but if failed then it is important to correct them immediately and not reinforce the response. Though many answers are yet to be answered, it is better to use errorless learning with the clients rather than using the trial-and-error learning, to help them cope in everyday life.\n\n***A Holistic Approach-*** Holistic program addresses the emotional problems along with the cognitive deficits. Many times, the individual with cognitive deficits face psychological problem, like depression. Such programs feature both individual and group therapy, embedded with goal planning approach to reduce emotional and cognitive problems. Long-term goals are those, which should be achieved by the time of elimination of rehabilitation process, whereas short-term goals are the steps taken to reach to long-term goals.\n\nEach long-term goal would require a short-term goal. It is necessary to address cognitive and emotional difficulties. It is required to involve the patient and the family in the process of decision-making. The goals should be such, which are practical and realistic and achievable.\n\n“Intensive, holistic, cognitive rehabilitation is an effective form of rehabilitation particularly for people who have previously been unable to resume community functioning.”\n - Cicerone",

| title | Memory- Encoding. Storage. Retrieval. |
| body | Memory is subdivided into three processes of *encoding, storage and retrieval*. For memory to function properly, we take information (encoding), store it or represent it in a certain way (storage) and then later recall it (retrieval). *Encoding* is a way through, which the information is processed for storage in the memory. Some of the information gets processed automatically, whereas for some we have put conscious effort. Eg- when you are listening to the lecture in class, you encode whatever the professor is saying, most of the times. For encoding, it is necessary that we attend to the information (Posner & Rothbart, 2007). Not just attention, but also the level of processing affects our memory. *Storage* encompasses how the information is retained over the period of time and how it is represented in the memory. According to Atkinson-Shiffrin’s theory, memory storage involves three systems-(a) Sensory memory: Through our different senses, we perceive the information. It is rich and detailed but the information in it is not retained for long. (b) Short-term memory/ Working memory: From sensory memory, the important information goes to short-term memory. It is a limited-capacity memory system, in which information is retained for around 30 seconds unless it is rehearsed. The concept of working memory is a workbench where information is manipulated. It helps us to understand how the damage to the brain affects our cognitive skills. (c) Long-term memory: It is a permanent type of memory, which stores huge amount of information. *Retrieval* is when the stored memory is recalled. The memory is taken out of the storage. It usually takes few seconds to search for the memory and retrieve it. It is a complex and sometimes an imperfect process (Dodd, Castel & Roberts, 2006). **Terminology** - **Implicit and Explicit memory**. Implicit memory in which that is expressed through physiological or behavioral changes when the individual has no or limited consciousness of the information being stored. Whereas explicit memory is when the memory is consciously accessed and it covers most standard memory tasks. - **Semantic and Episodic memory**. Semantic memory is where the concepts or facts are culturally or educationally acquired, eg- general knowledge. Episodic memory is information encoded, stored and utilized, through the personally experienced events. - **Recent and Remote memory**. Recent memories are those, which are stored within last few hours, days or weeks. Remote memories date from childhood till present. - **Anterograde and Retrograde memory loss**. After a neurological illness/ injury, it is difficult for an individual to acquire new knowledge and remember the recent events. In retrograde, the memory for events before the onset of illness/injury is affected. - **Prospective memory.** It requires interaction between episodic and executive functions, to enable an individual to remember something, at a particular time in future. - **Material specificity.** Memory for verbal and non-verbal material is affected differently, in individuals, as different hemispheres encode different information. Encoding/ retrieval differences may be hemisphere specific as other memory processes (Kennepohl et al. 2007). - **Recall and Recognition.** Recall is the uncued retrieval of the information and involves active search process. Whereas, recognition is when the individual feels familiarity with the recall component, from many other stimuli. In my next blog, I would be writing about memory disorders and their rehabilitation. |

| title | What is Memory? |
| body |
"Memory is more indelible than ink." - Anita Loos Why do we remember our name? Where we live? What is your parent’s name? What is your friend’s name? The simple answer is because we have something called memory. Memory refers to the processes, which are used to acquire, store, retain and later retrieve the same information. Memory is a faculty, wherein the sensations, impressions, and ideas are stored and then recalled. Our brain works like a computer. Initially, the brain puts the important information into ‘files’, and when required or when you remember something, that file opens. Memory is one cognitive function; we use the most in our daily routine. We use memory to store different types of information, for eg- an appointment, phone number, what you had for breakfast, which movie you saw last…etc. Also, memory plays a role in other activities like reading, calculations, reasoning, etc. Sometimes we use memory without being aware that it is being used. Memory is an integral part of our existence still, it is vaguely understood. In my next post, I would explore some important terms related to memory. If you guys want me to write on something, in particular, do let me know. I would be more than happy to write about it! |

| title | Treatment for Depression |
| body | Depression is the most common mental disorder. And,various treatments are available for depression. **Medications**- Many antidepressants are available like selective serotonin reuptake inhibitors (SSRIs), serotonin and norepinephrine reuptake inhibitors (SNRIs), atypical antidepressants, tricyclic antidepressants, monoamine oxidase inhibitors (MAOIs), etc. But, discuss the major side effects with the doctor before starting medications. **Psychotherapy**- It is an effective treatment for depression. Many studies support that; therapy can be a powerful treatment for depression. Different types of therapy- *Cognitive therapy, behavioral therapy and cognitive behavioral therapy. Interpersonal therapy*- it focuses on your relationship with other people. *Psychodynamic therapy*- it explores the motivations that you are not aware of, which is contributing to feeling depressed. *Individual counseling*- is a one-on-one session where therapist teaches you more about depression. *Family therapy*- it helps to improve communication in the family and that helps to resolve the conflicts. # When a loved one is suffering from depression, the family and friends support plays a crucial role in returning the well being of the individual. Providing emotional support is very important.
**Some suggestions for successfully handling the situation are:** ***Provide emotional support…*** Listen to your loved one without judging and show that you understand what they are going through, show patience, affection, and encouragement. Coping with depression can lead to anger and frustration but be calm and reassure him/her that everything will be fine. Make sure that person suffering from depression knows that you are there for them. **Help the patient to get an appointment…** The person suffering from depression needs professional help to recover. The diagnosis and treatment is an important part of the intervention. Practitioners, who have specialized in treating mental health disorders, can provide the treatment. This includes licensed psychiatrists, psychologists, professional counselors and social workers. So, take him/her to the professional and help them to get better. **Cultivate a “we’re in this together” attitude…** An individual struggling with depression can feel completely alone. So, try to do activities together and help them to feel less lonely. Do activities together like, go for a walk together or go watch a movie, being with the sufferer will make them feel good about themselves and they will feel as if someone is there for them, who care for them. **Reach out to others…** Support groups exist for friends, family members/ caregivers of people struggling with depression. Explore the help and support that might be available through local social services or counseling, which will help you to better manage the needs of the sufferer and yourself. Seeing that other caregivers suffer through same problems as yours, then it gives you some relief, that you are not alone. **A Note about Suicide…** Because of the high suicide risk, it’s important to note the warning signs. In addition to those mentioned in the depression, other signs include: Withdrawing from loved ones and isolating oneself, talking or writing about death or suicide, putting personal affairs in order and previous attempts. Take the signs seriously. Seek professional help. |

| title | Depression- Types, Symptoms and Signs |
| body |
In my last post I talked about what is depression and some facts related to depression. Depression is a psychological disorder, which negatively affects how the person feels, think and act. It causes feelings of sadness and/ or loss of interest in activities once enjoyed. Depression is of different types according to nature of the illness. The most common types are depression are- - Major Depressive Disorder- it is a more severe form of depression, than other types. - Dysthymia- It is a long-term depression. It is less severe but persists for two years or more. - Atypical Depression- Key symptoms are overeating, oversleeping, fatigue and severe mood swings. In typical depression there is pervasive sadness and loss of appetite and sleep. - Bipolar Depression- Also called as manic depression. There is alteration between periods of clinical depression and extreme elation or mania. Causes of depression are varied and it is important to figure out the cause as it helps in planning treatment of the disorder. - Genetic factors- depression is said to run in families. Researchers are trying to find genes that may be involved. - Hormones- Changes in the hormonal balance may trigger depression. Changes in hormones can be due to thyroid problems, menopause, etc. - Life events- Traumatic life events such as loss of a loved one, academic stress, financial problems or any childhood trauma can trigger depression. - Biological differences- People with depression have physical changes in their brain; it may help in pinpointing the cause. - Brain chemistry- Neurotransmitters (brain chemicals) imbalance, may cause depressive symptoms. Some of the common signs and symptoms of depression are- 1. Feeling anxious, sad and ‘empty’ persistently. 2. Feelings of hopelessness. 3. Restlessness or irritability. 4. Feelings of worthlessness, guilt or helplessness. 5. Fatigue or tiredness. 6. Loss of interest in normal activities. 7. Slowed thinking and speaking. 8. Change in appetite- can be reduced or increased. 9. Trouble concentrating and remembering things. 10. Sleep disturbance- generally insomnia. 11. Unexplained physical problems. 12. Frequent thoughts of death, suicidal thoughts. Sometimes suicide attempts. Depression can affect individuals with any age and symptoms of depression in children and teens are bit different, mostly the symptoms are same to those of adults’ but some differences are- - In younger children symptoms may include clinginess, aches, refusal to go to school, being underweight, etc. - In teens, symptoms may include sadness, irritability, feeling negative, poor performance or poor attendance at school, feeling misunderstood, using drugs or alcohol, self-harm, avoidance of social interaction, etc. My aim of the post is the spread awareness about depression. In my next post I will be talking about the treatment options for depression. |
