A review published ahead of print in the American Journal of Physiology-;Gastrointestinal and Liver Physiology explores newer, potentially safer therapies for treating chronic abdominal pain with lower risks of addiction and side effects. Opioid drugs treat pain by binding to specialized nerve cells (called opioid receptors) in the brain and other parts of the body, including the gastrointestinal tract and spinal cord. Long-term use of opioids for chronic, generalized abdominal pain-; also called visceral pain-; can change the way the opioid receptors work, leading to addiction. Opioids can also cause respiratory depression-; slowed breathing and a reduced ability to properly inhale and exhale-;which can be fatal if not treated quickly. IBS, pancreatitis and visceral pain may not respond to non-opioid painkillers or medications called neuromodulators, such as antidepressants. More and more, doctors have been prescribing opioid drugs for these challenging gastrointestinal conditions. "From 1997 to 2008, opioid prescriptions for chronic abdominal pain more than doubled in U.S. outpatient clinics," wrote the review's author, Michael Camilleri, MD, of the Mayo Clinic. The main mechanism that opioids use to reduce pain is through stimulation of the Mu-receptors. New approaches for treating abdominal pain include drugs that pinpoint different types of pain receptors, not only opioid receptors. In addition, newer generations of opioid medications may be able to relieve pain without the risk of addiction or respiratory depression. Potential new treatments include: Oliceridine, a drug that binds with a specific component of the Mu-opioid receptor without activating the part of the receptor that causes severe constipation or breathing to slow down. For others—like those who suffer from conditions like endometriosis or adenomyosis—the pain can be severe, chronic, and even debilitating. But cramps and abdominal discomfort aren’t always period-related, and you should consider other causes if you have ongoing pelvic pain. That’s especially true if your cramps don’t get better or worse around the same time every month. RELATED: 5 Period Side Effects That Aren't Cramps Talking to your gynecologist is a good start, but you may also want an evaluation from your primary care physician, a gastroenterologist, a urologist, or a pelvic medicine specialist as well, says Benjamin Brucker, MD, assistant professor of urology and ob-gyn at NYU Langone Health. “There are several different categories of health problems, such as pelvic-floor issues or bladder issues, that could contribute to a similar type of pain,” says Dr. Brucker. If you suspect your persistent symptoms aren’t related to your monthly cycle, here are some other potential culprits. Kidney stones Kidney stones—hard masses formed in the kidneys from calcium—often cause back or side pain just below the ribs. But if a stone works its way down the urinary tract, it can cause pain in the lower abdomen or even the vagina. An ultrasound or CT scan can help diagnose this common condition, and blood and urine tests may also be required to rule out other problems. Small kidney stones can usually be passed on their own with the help of over-the-counter pain relievers and plenty of fluids. Doctors may also prescribe medications known as alpha blockers, which help relax the muscles in the urinary tract. Large stones may need to be broken apart with sound-wave technology or removed surgically. There's no diagnostic test for painful-bladder syndrome, and treatment varies depending on a woman’s specific symptoms. One potential therapy include electrical stimulation of the pelvic nerves, a treatment also used for overactive bladder. A fibroid, cyst, or tumor If you're experiencing unexplained pelvic pain, doctors will often perform imaging tests to look for structural causes—like uterine fibroids, a cyst on the ovaries, or, rarely, a tumor in the reproductive organs. Gas It’s no secret that extra air in the digestive tract can cause bloating and cramping, and this often occurs after eating gas-producing foods like cauliflower, beans, and broccoli. Some people also suffer from chronic bloating, which may have to do with a bowel disorder, a food intolerance, or even the way they breathe. “When you have chronic bloating, the air pushes forward and stretches your abdominal wall and makes it hurt,” says Dr. Dugan. Under the umbrella term, diabetes, there are two common types. Endocrinologist, Dr. Sumyra Mehkri says Type 1 diabetes is genetic. It’s when the body does not make enough insulin to support the body. Type 2 diabetes is where your body does not use insulin properly. After practicing for more than 20 years, Dr. Mehkri says both types can present themselves with a multitude of symptoms or sometimes none at all. “Some, no symptoms at all and they just come to the doctor and they tell the doctor, I need a checkup and they find out that they're diabetic,” said Dr. Mehkri. “Because we’re not aware of all these things it takes a long time and before the patient even seeks attention from a doctor he's already been diabetic maybe for the past three years.” Dr. Mehkri says it’s important to be alert to the symptoms. She says some of the symptoms include, “thirst, increase thirst, have a lot of hunger, feeling very tired, very fatigued, sometimes abdominal pain.” The doctor says you should also look for a sudden change in vision, numbness in the feet or hands and headaches. Both Type 1 and Type 2 can come with different symptoms. “Type 1 people, because it’s a more severe kind of diabetes because they have no insulin in their bodies, they can just start with a severe abdominal pain,” said Dr. Mehkri. “They could just come as like severe infections, severe weakness, weight loss, that is Type 1.” There are other rare symptoms of Type 1 diabetes such as sepsis, which is a severe infection throughout your body and losing consciousness. “They have to get their eyes checked once a year, their feet checked once a year, their heart check once a year and the blood sugars checked depending on how well they’re controlled,” said the doctor. The most common symptom of Type 2 diabetes is frequent urination, getting up two or three times to go to the restroom every 30 minutes to an hour. Dr. Mehkri says the best thing you can do is educate yourself about the disease and know the symptoms. Get tested for diabetes if you experience one or several of the symptoms mentioned above. “If the symptoms are milder and they don’t feel that sick then they should come to a doctor and get checked especially if it runs in their families,” said Dr. Mehkri. She says the challenge is education about the disease among the public. | body | March 27 (UPI) -- About 90 percent of people in the world with hepatitis B are unaware they are living with the condition, and just five percent are receiving necessary medical care, according to new research. About 291.9 million people are infected with hepatitis B -- 3.9 percent of the world's population -- according to a study published Monday in the Lancet Gastroenterology & Hepatology journal. The data came from Polaris Observatory, which participated in the research and regularly reports more generally on efforts to curb hepatitis. Hepatitis B, an infectious disease, is primarily transmitted through contact with the bodily fluids of an already infected person. It is a leading cause of liver cancer, according to the Centers for Disease Control and Prevention, with 4 million people infected with it -- most of whom are unaware. No virological cure exists for hepatitis B, but a vaccine has been available since 1981. The researchers developed models for 120 countries, 78 of which were populated with data approved by experts. Only 22 were estimated to have already reached the 2020 target of diagnosing one-third of the infected HBV population. "Although some regions are well on their way to meeting prophylaxis and prevalence targets, all regions must substantially scale-up access to diagnosis and treatment to meet the global targets," the researchers wrote. Around 1.8 million children aged 5 years of age were infected. The WHO recommends that all newborns receive their first dose of a vaccine within 24 hours of birth, part of its wide-ranging Global Health Sector Strategy to eradicate the disease by 2030. Countries on track to eliminate hepatitis B among 5-year-olds by 2030 include the United States, Australia, China, Japan, France, Germany and Russia. Britain is on track to reach less than 1 percent by 2030. If left untreated, HBV can cause serious diseases of the liver, including cancer. An estimated 600,000 people die every year from hepatitis-B related liver diseases, making HBV a bigger killer than malaria. A test for HBV has been available since the early 1970s, but only one in ten sufferers worldwide have been diagnosed. The virus is highly contagious via infected blood or other body fluids and is mainly transmitted from mothers to their infants, or between children. There is no cure, but antiviral drugs have proven effective in coping with symptoms. A vaccine against HBV became available in the early 1980s. Since 1992, the World Health Organization has recommended the first dose within 24 hours of birth, but only half of the newborns are vaccinated that quickly. "Most mother-to-child transmission occurs within days of birth, so the birth dose is vital," said lead investigator Homie Razavi, a virologist at the Center for Disease Analysis outside Denver, Colorado. Endometriosis affects approximately one in 10 women, so it’s crucial to recognize the signs and symptoms of this illness — especially because we are so often told that this type of pain is simply part and parcel of being a woman. HelloGiggles spoke with experts who told us about seven endometriosis symptoms to look out for. 1  pain “Pelvic pain is the most common symptom associated with endometriosis,” Dr. Amy Schutt — a reproductive endocrinologist at Texas Children’s Family Fertility Center at Texas Children’s Pavilion for Women in Houston — told us. The pain can be cyclical and often worsens with menstrual periods or activity such as sexual intercourse. 2 Dymenorrhea Dr. Jessica Ritch, Minimally Invasive Gynecologist at Florida Center for Urogynecology, said that dysmenorrhea (pain with menses) typically starts during the teenage years or early 20s and gets progressively worse as time goes on. 3Dyspareunia Dyspareunia is the medical term for painful intercourse. “This can be the pain with initial insertion, but more often is a pain with deeper penetration,” Ritch explained. “It can also continue after sex.” 4Changes with urination or bowel movements Although these are less common signs of endometriosis, Schutt said some patients experience changes with urination (such as pain and increased frequency) or bowel movements (constipation and diarrhea). 5Infertility Even if they’ve never had the painful symptoms, Ritch said that some women with endometriosis have difficulty getting pregnant. 6Back pain Both Schutt and Ritch noted that back pain is a symptom of endombackward Ritch explained this is especially common when the uterus is retroverted (tilted backwards). 7Shortness of breath, chest pain, and cough In rare cases, endometriosis can affect distant organs like the lungs. In these cases, women experience shortness of breath, chest pain, and coughing. “This can appear like pneumonia or even cause women to cough blood,” Ritch said. | body | A file photo of skin cells. Modern research technology has led to the discovery of a long-misunderstood organ that could be full of answers to questions doctors have had for centuries. The organ, known as the interstitium, is the largest organ in the body and is full of fluid that cushions organs. Scientists had a feeling such a structure existed for years but were unable to study it due to the limitations of previous medical tools. A new article in Scientific Reports is the first to classify the structure as an organ and explain how it works. Neil Theise, a pathologist at New York University studying the interstitium, has found a potential link between the organ and the ways that certain types of cancer spread. "It's a fluid cell highway," he explained. "When [cancerous cells] get into this layer, that's when they spread." Cancerous cells attack the body by rapidly multiplying and are at their most deadly when they reach the lymph nodes. By this point, treatment becomes very difficult and chances of survival decrease. It's ideal to catch the mutated cells before they reach the lymph nodes, but some types of cancer, such as urinary cancer, are not possible to screen. Theise hopes that if the cells are entering the bloodstream via the interstitium, they may leave a trace of protein in the fluid surrounding much of the body. Doctors can sample this fluid and test for cancerous cells before they reach the lymph nodes and become terminal. Research on the interstitium still has a long way to go, but Theise believes that many answers lie in the research to come. The organ's structure and functionality may help researchers learn why certain cancers — such as skin, breast and stomach — spread and help doctors treat aggressive cancers. The research may also help explain other medical mysteries, such as the effectiveness of acupuncture and where lymphatic fluid originates. One Of The Deadliest Cancers Is On The Rise. Why? While pancreatic cancer only accounts for about three percent of all cancers in the U.S., according to The American Cancer Society, it's often considered to be one of the deadliest. Currently, pancreatic cancer stands as the third-leading cause of cancer death in America. | body | March 28, 2018 A new study indicates that patients with alcohol-related liver cancer often do not live as long as patients with liver cancer that is not associated with alcohol consumption, mainly due to diagnoses at later stages. Published early online in CANCER, a peer-reviewed journal of the American Cancer Society, the findings indicate that efforts should be made to improve both screening for early signs of liver cancer and the management of alcohol abuse. Liver cancer is the second leading cause of cancer-related deaths worldwide, with hepatitis B and C infections being the main causes. Alcohol abuse and non-alcoholic fatty liver disease are other dominant risk factors. Due to improvements in the treatment of hepatitis infections and increased alcohol consumption in some regions, it is likely that alcohol will become a leading cause of liver cancer in the near future. Indeed, alcohol is already the first cause of liver cancer in France and involved in 25 percent to 30 percent of diagnoses in the United States. The real US figure is likely higher as alcohol consumption is often underreported when another risk factor is present. To compare aspects of alcohol-related and non-alcohol-related liver cancer, Charlotte Costentin, MD, of Hôpital Henri-Mondor in France, and her colleagues examined 894 patients with newly diagnosed liver cancer who were followed for 5 years; 582 patients (65 percent) had a history of chronic alcohol abuse and 312 (35 percent) did not. Investigators also recorded whether patients with alcohol-related liver cancer were abstinent or not at the time of cancer diagnosis. A total of 601 patients had died by the time of the investigators' final analyses. Alcohol-related liver cancers were more likely to be diffuse and were detected in patients with worse liver function. Median overall survival was 9.7 versus 5.7 months in the non-alcohol-related and alcohol-related groups respectively. When investigators looked at each stage of cancer individually, however, survival was similar in patients with non-alcohol-related and alcohol-related cancer. The findings suggest that patients with alcohol-related liver cancer have a reduced overall survival mainly due to worse liver function and tumor characteristics at diagnosis. The analysis also examined whether patients were participating in cirrhosis follow-up programs before their cancers were diagnosed. (Most people who develop liver cancer show signs of scarring, or cirrhosis, in the liver, and international guidelines recommend ultrasound every six months to detect early liver cancer in patients with cirrhosis.) Patients whose liver cancer was detected during a cirrhosis follow-up program had improved survival compared with patients whose cancer was diagnosed incidentally. This was especially pronounced in patients with non-alcohol-related liver disease or those with alcohol-related liver disease who are no longer drinking alcohol compared with non-abstinent patients. Also, non-abstinent alcoholic patients had the lowest survival in the study, even when restricting the analysis to patients involved in cirrhosis follow-up programs. "To improve prognosis of liver cancer in the alcoholic population, efforts should be made to implement effective screening programs for both cirrhosis and liver cancer, and to improve access to alcoholism treatment services," said Dr. Costentin. "A smaller tumor burden and a better liver function at diagnosis should translate into higher rates of patients with alcohol-related liver cancer amenable to curative treatment such as tumor resection or ablation and liver transplantation." |

| body | When thinking about how to treat mesothelioma cancer, most of us don’t consider the spice rack. Maybe we should — beginning with turmeric. Turmeric is a deep golden-yellow spice used in Asian and Indian dishes. In addition to its culinary role, turmeric has been used for thousands of years as a traditional medicine. Research over the past few decades supports turmeric’s anti-cancer activity in the lab. Now, a 2017 study shows turmeric may have anti-cancer activity against mesothelioma cells, too. This focus on using turmeric against many types of cancer shows the spice may be good medicine indeed. Turning Turmeric Against Mesothelioma There are two aspects to the 2017 turmeric and mesothelioma experiment. In both parts, the researchers focused on curcumin, a substance found in abundance in turmeric. First, they studied how curcumin curbs cell growth in mesothelioma cancer. The researchers found the spice component thwarted mesothelioma by: Increasing the damage due to oxidation in mesothelioma cells — oxidation is one way some treatments kill cancer cells Turning on a type of cancer cell death, called apoptosis, which helps the body combat cancer Improving activity in mesothelioma cells, called autophagy, which is associated with more cancer cell death Turning down the ability of cancer cells to survive when damaged Next, researchers examined how curcumin protected mice against mesothelioma tumor development. They placed mesothelioma cells into each mouse’s body through the peritoneum. The peritoneum is the membrane lining the abdominal cavity. It encloses organs, such as the stomach and intestines. Peritoneal mesothelioma is one type of the disease affecting humans. Half the mice received curcumin dissolved in corn oil in their abdominal cavity along with the mesothelioma cells. The other half received plain corn oil with the cancer cells. This type of treatment delivery is called intraperitoneal administration. This is similar to how a chemotherapy treatment called HIPEC is delivered. HIPEC can be used to treat peritoneal mesothelioma. Compared with the mice who received plain corn oil, the mice who received the intraperitoneal curcumin and corn oil mixture: Experienced reduced risk of developing tumors overall Developed fewer tumors Had much smaller tumors Lived significantly longer The study authors noted this information is important to consider when planning future studies of curcumin to treat mesothelioma. Animal studies do not prove a treatment is effective in people, and this animal study is no different. However, it does provide vital clues regarding how a human study with curcumin may need to be designed. Taking turmeric orally may not work, but combining it with other treatments delivered straight to the tumor may improve tumor control. A Spicy Solution for What Ails You Turmeric has a savory, warm flavor, and it is a staple of many Asian and Indian recipes, including Indian curry dishes. For thousands of years, the spice has been used to treat a range of conditions, including intestinal gas, worms, gallstones, arthritis, menstrual problems, cuts, bruises, irritable bowels, skin infections, asthma, diabetes, coughs, colds and sore throats. Modern medicine has taken notice of turmeric, too. More than 3,000 scientific publications on the spice have been published in the past 25 years. Many have focused on turmeric and cancer. Factors to Consider When Using Turmeric or Curcumin Turmeric and curcumin are available as dietary supplements. These substances are generally considered safe for most people. However, you should not take any dietary supplements or over-the-counter medications without first discussing it with your mesothelioma health care team. Some dietary supplements can interfere with mesothelioma treatment. Only your oncologist and you, working together, can decide if a supplement is safe for you to use before, during or after treatment. Supplements can interact negatively with your treatments and other medications. These interactions can worsen side effects or render your medical treatments less effective. A recent medical paper demonstrates in most cases, it is not a good idea to take alternative medicine in place of conventional treatment options such as surgery, radiation therapy or chemotherapy. Cancer patients who opt to use alternative medicine in place of conventional therapies have shorter survival compared with patients who do use mainstream treatments. Finally, the digestive tract does not easily absorb curcumin on its own. Only supplements that help the body absorb curcumin are likely to make their way into your body. When taken by mouth, pure curcumin may not reach your tumor effectively. Ask for What You Need If you are being treated at a large cancer center, ask your mesothelioma doctor if there are any supportive therapies available to you through their facility. You may be able to speak with a dietitian for nutrition consultation, try acupuncture or massage, or pursue other options to manage symptoms and improve your quality of life. If these services are not available, let your doctor know if you plan to pursue them on your own. When there is complete absence of sperm, it is called azoospermia.Low sperm count, according to an obstetrician and gynaecologist at FT Healing Medical Centre, Lagos, Dipo Olamekan, is inseparable with fertility. He said when a couple is trying to have children, the number of sperm the man ejaculate is quite important in the whole process. When it is lower than 15 million, there is a problem. He said all hope is not lost, as there is medical solution to the challenge. Common causes of low sperm countA report on mayoclinic.org said “the production of sperm is a complex process and requires normal functioning of the testicles (testes) as well as the hypothalamus and pituitary glands — organs in your brain that produce hormones that trigger sperm production.”The report said “once sperms are produced in the testicles, delicate tubes transport them until they mix with semen and are ejaculated out of the penis. Problems with any of these systems can affect sperm production.”Dr. Olamekan said while the average man could produce the much-needed sperm multiple times per day, sometimes an internal factor, such as varicocele, could lower the number of sperm.According to him, varicocele might develop as a result of poorly functioning valves that are normally found in veins, and might also occur from compression of a vein by a nearby structure.Said he: “Note that most of the factors that decrease sperm count are external factors such as smoking, drug usage, poor diet and lack of exercise. In addition to that, heat can have a detrimental effect on normal sperm production. It is for this reason that the testicles are naturally placed outside the body in the scrotum rather than in the abdomen, like the ovaries. Soaking in a bathtub full of hot water can almost halt sperm production completely.“Obese men can also become sterile because the sagging layers of fat can overheat the testicles.“Men whose jobs involve long hours of sitting are at risk. Office workers who don’t move about and long distance lorry-drivers may have infertility due to the increased heat to the genital area.”The gynaecologist also warned that frequent intercourse could result in the demand exceeding the supply. He warned that men who ejaculate 2-3 times a day could easily develop low sperm count.The medical doctor revealed that cigarette smoking of more than 20 sticks a day was capable of reducing both the sperm count and sperm motility. He added that excessive alcohol intake could lower the production of sperm and of the male hormone testosterone.He said: “A workaholic husband may find that stress and fatigue can potentially interfere with sperm production.”Meanwhile, in analysing sperm count, the specialist described it as a complex process, saying that there were a number of measurements considered in semen analysis.“A physician will look at the concentration – how many sperm there are per cc or millilitre of semen (the fluid). Another measurement to consider is what percentage of the sperm is moving forward progressively; 50 per cent motility is considered normal.“The next factor is sperm morphology, which determines what percentage of sperm look normal. When you assess the fertfertilizing of a given specimen, you must consider all these factors.”Some infections could interfere with sperm production or sperm health and thereby causing low sperm count. Inflammation of the epididymis (epididymitis) or testicles (orchitis) and sexually transmitted infections, like including gonorrhea or HIV, are some infections that could hamper sperm count.Ejaculation during sex could lead to low sperm count if it does not occur the normal way. Normally, when a man ejaculates, semen comes out of the tip of the penis, following a tube or passage. But for low sperm count caused by infections of the urinary/reproductive tract, he prescribed antibiotics to clear the infection, while medication and hormone replacement treatment options could be used when the cause of low sperm count is related to a hormone imbalance.“In cases of low sperm motility, assisted reproductive technology (ART) treatments would be used to facilitate conception by inserting sperm into the female reproductive tract,” he stated.
