| body | Well we are home! What a beautiful trip. 3 weeks is a pretty long time in my opinion to be gone from home. I missed home, I missed my bed, my satin sheets, a big shower stall, my closet and most of all my plants. As soon as we got home, once we helped hubby park the RV, unhook the Jeep from it, I rushed to my kitchen garden. One of my friends, Ryan, has been tending to it while I was gone. He helps me plan the space, till the ground, get fertile soil from compost facility and then plant it too. I returned to lots of zucchini, chillies, beets, beans, cucumbers, green tomatoes, gladioli flowers that we planted last year and a few others. My herb garden also looks super good. I really need to start drying and storing them for winter. This makes me happy! My work Christian loubs, pretty shoes make me happy! |

| body | Our boys are in junior high, I have always believed kids need in classroom education. From feb2020- end of yr that’s June, they stayed virtual and finished the yr. teachers and kids were still learning what it means to be virtual. So the last quarter was basically no studying just going through the routines. School took summer of 2020 to understand this new normal form of education- hybrid. Some kids still chose to stay hybrid and most returned to school with masks and armed with their sanitizers. Lunch was behind plexiglass separators. Not ideal but still better than virtual. I think socially this last year has stunted them emotionally, although our boys are twins and depend on one another for their social well-being, they still had night outs with their friends, which stopped. No picnics or any school trips. But all in all I think we did quite well as parents in shielding them from misinformation and panic associated with COVID. School did quite well in adapting to a hybrid model to bring kids back into school in a very reasonable quick manner. |

| body | I am not sure if we can truly call the world of today post-covid just yet. But roll with it with me, will ya? America and Americans are very gregarious, social and fun loving people and spend most of their time in company of others. Asking a people like that to stop living their life like they have lived for centuries and adopt a new way has not been easy. So once CDC recommended vaccinated people can start to get together socially, first in outside settings and later inside too, has been a welcome gesture. I find myself in a unique position- I am a physician- first responder, a mom- kids in junior high, business owner- I own a restaurant, hospital administrator- I am the chief of finance of my health group. I will try and present my POV in each setting. Physician- 2020 was a year where many of our patients and society in general appeared to be in awe of us and the work we do. Our job had always been dangerous, I remember during HIV and AIDS pandemic, hubs and I were always worried about a needle stick, we were still in Med schools and then residencies, planning a family. Our jobs required taking care of trauma patients with multiple gunshot wounds in south side of Chicago- notorious for gang violence. Life was dangerous, contracting HIV was a real possibility and it was considered a long slow death sentence. Fortunately we have some great treatment options now. I don’t remember general population caring or understanding the risk of being in medicine in those days. COVID has been an interesting disease, people have been very informed and disease has stayed in news on a daily basis- 24/7. I am not sure if it was due to political reasons , the dramatic press conference our then president conducted or stay at home orders. As we have made our way into 2021, a fatigue has set in and a general “I don’t care anymore” attitude is quite apparent. I have noticed a general increase in impatience, care on demand and feeling of entitlement. Social factors and frustration I understand has a lot to do with this human behavior. Mental health issues are now my most common chief complaint that needs to be addressed. I have referred more patients to counseling and started on anti anxiety/ antidepressants than ever before. I pray this is temporary and not a start of drug addiction and suicide/ homicide pandemic in US/ the world. Another aspect is medical conditions that were left untreated due to scare of COVID, are now to the point of either being very advanced requiring multiple surgeries or becoming non treatable. That saddens me the most. |

| body | March 2020- 2nd Saturday - I still remember that to be the last Friday where my world was “normal”. Thing 1 and 2 had built a robot and programmed it themselves to do a few tasks. Hubs and I planned a whole day with them at University of Iowa followed by lunch and cupcakes. Next day our spring break started and we drove to lake of Ozarks in Missouri for a camping trip. I still vividly remember receiving message after message on Sunday from my work about clinic being cancelled. Emergency planning in place, I heard the word N95 for the first time in my life, mask mandate and face shields. Executive sessions back to back discussing work flow, consolidating clinics, closing clinics. Being in a campground with very limited internet, we really didn’t grasp what was happening in the country within span of a weekend. We cut short our trip and returned home. Hubs being hubs- smart, forward thinking plan B kinda guy, order N95 masks from different manufacturers for us, our partners at work and staff. We returned to a new way of working and delivering care. I made a list of my most vulnerable patients and started calling them along with my staff. The worst were considered to be patients with bad lung disease like bronchitis, severe asthma, cancers, low immunity patients, uncontrolled diabetic patients. No one knew anything about the disease, everyday a new symptom and a new treatment, symptoms stayed but treatment refuted, leaving us with no options to help. I remember the feeling of helplessness. In my career I have never sent a patient with a fever of 105 home. An acceptable oxygen saturation in a human blood is 95-100%, I had never heard of oxygen levels falling into 60’s 70’s in otherwise healthy people. This just doesn’t happen. We as physicians weren’t trained to tackle this in our outpatient clinics. Hospitals weren’t accepting patients, sending people home- only acceptable reason for admission was requiring intubation. That means- fever, cough, inability to keep anything down- all sent home. No testing was available, took 14 days for results to be delivered. What was the point by then? Over the next few months, some logical treatment options became available, then the vaccine , then convincing people to get vaccinated, that there is no transmitter in the vaccine to track them. Seriously who comes up with this nonsense? Through the year, I lost two patients to COVID- both were sick. I take pride in the fact that my team stayed very proactive and called our patients with chronic diseases frequently and kept a close eye on them. I do regret that a very sweet 82 year old patient of mine developed a spot on his ear and didn’t really disclose it to anyone until it became a large incurable lesion. The consequences of delay in seeking care for non covid related illnesses started to become apparent in year 2021. Most older patients had not seen their grandchildren in over a year, this has had a huge detrimental effect on their physical and mental Health. Patients in long term facilities were sequestered and no family was allowed to visit them, leaving them completely isolated. By March and april 2021 most of my 65+ patients had received the vaccine- either Pfizer or Moderna. I see hope in their eyes of end to this new normal. Hope that they will be okay. Now starts the post covid USA. |

| body | March 2020- 2nd Saturday - I still remember that to be the last Friday where my world was “normal”. Now starts the post covid USA. |

| body | Saying goodbye to Edison, we travelled further east towards Maine. Or atleast that’s what we thought we were doing. Yours truly felt a need to go see the majestic Niagara Falls yet again. Something about these falls is very attractive and as expected the place didn’t disappoint. Fragrance of flowers, sound of rushing waters, singing of birds… calms your heart and mind for those few minutes. Falls still are huge, mist upon them still hanging over the falls- 24/7/365. We stayed at the falls till lights came on. The scene at night changes so drastically, the soft hues of colors in the rising mist, darker shades of water right underneath the fog are mesmerizing. Hubs and I ended this day with a celebratory glass of wine for a day well spent in love and happiness, while making some new memories with our boys. |
