      "body": "Otocephaly is characterized by agenesis or severe hypogenesis of the mandible or agnathia, synotia (the external ears horizontally placed and/or fused), microstomia (“small mouth”), aglossia (congenital absence of the tongue). This anomaly is often associated with other malformations as; Holoprosencephaly, anencephaly, congenital heart\ndisease, tracheoesophageal. \nThe etiology is unknown. The pathogenetic mechanism seems to be related to a complete failure of the mandibular development, possibly associated with inhibited or arrested migration of neural crest cells. Three-dimensional ultrasound is often the only way to obtain an overall idea of the anomaly. The virtually all views of the fetal face are abnormal, due to the complete distortion of the facial anatomy. Otocephaly has been always lethal, so when detected, an accurate counseling of pregnancy should be offered at parents with also the possibility to offer termination of pregnancy. We report the case of a fetus, who was born on the 30th week of pregnancy, and was diagnosed with otocephaly, after birth.\n\nA 21-year-old G2P1A0 patient presents at 30 weeks gestation for labor with uterine contractions, and is found to be in active labor with the fetus in breech presentation. Clinical assessment of the maternal pelvis is determined to be adequate for a fetus of this estimated weight.\nThe cervix is 10 cm dilated, 100% effaced and the fetal pelvic are at -3 station. Ultrasound examination reveals of polyhydramnios, was reconfirmed afterwards with rupture of\nmembrane he had approximately 5 L amniotic\nfluid. The patient had no medical and family history of any congenital anomalies, and likewise had no antenatal history of infection/radiation or toxin exposure. She has had a non-good prenatal care, including lack of one anatomy scans in the second trimester. Her prior infant, weighing approximately 3500 g, was delivered vaginally without complications. He was completely healthy. After 30 minutes, the patient with vaginal delivery, with manual assistance, according to Bracht maneuver, gave birth to a male baby with body weight 900 g and Apgar score 4/5. The macroscopic placenta was normal, and the umbilical cord with central insertion and that had three vessels. A detailed examination showed multiple anomalies of face as; Synotia (the external ears horizontally placed and/or fused), Agnathia (agenesis or severe hypogenesis of the mandible), Microstomia (“small mouth”), Aglossia (congenital absence of the tongue). Otocephaly ultrasound diagnosis can be made on the midsagittal view of the profile, where to all views of the fetal face are abnormal, due to the complete distortion of the facial anatomy. The fetal face represents an extremely irregular region to explore with ultrasound. This is why ultrasound examination of the fetal face has always posed great difficulty during the examination. We have some scanning planes, classified as axial, coronal, sagittal, and oblique, and the anatomic structures assessable in each of these views. In an axial view are usually seen; tongue, pharynx, Mandible, inferior alveolar ridge, mandibular bone etc. In a sagittal view are usually seen; facial profile (midline sagittal) and ear—parasagittal (lateral). The final scanning planes to complete the evaluation of the splanchnocranium are as follows:\n\n1) The coronal view of the face and palate;\n2) The oblique view of the upper lip;\n3) The coronal/oblique view of the palate. \n\nScanning for fetal anomaly is usually performed between 18 and 22 weeks’ gestation. Otocephaly has been always lethal, so when detected, an accurate counseling of pregnancy should be offered at parents with also the possibility to offer termination of pregnancy. In such cases, postnatal treatment is not possible. Our case was a malformation of order that consisted of synotia, agnathia, microstomia, aglossia. A case that after six hours end to death. In the pathogenic aspect , during the 4th weeks gestation, the mesoderm lateral plate of the ventral foregut region becomes segmented to form a series of five distinct bilateral mesenchyme swellings called the pharyngeal (branchial) arches. Ventrally migrating neural crest cells interact with lateral extensions of the pharyngeal endoderm, surround the six aortic arch arteries, and initiate pharyngeal arch development. The initial mesodermal core of each arch is augmented by neural crest tissue that surrounds the mesodermal core. The mesoderm will give rise to muscle myoblasts while the neural crest cells give rise to skeletal and connective tissues.\nOtocephaly: a case postnatal diagnosed Astrit Gashi et al.\nThe pharyngeal arches are separated by pharyngeal grooves on the external aspect of the embryo, which correspond internally with five outpouchings of the elongated pharynx of the foregut, known as the five pharyngeal pouches. Although derivatives of five or even six arches are described, only four arches appear externally. The first pharyngeal arch is the precursor of both the maxillary and mandibular jaws and appropriately bounds the lateral aspects of the stomodeum. Meckel’s cartilage arises at the 41st to 45th days post conception, provides a template for subsequent development of the mandible. Persisting portions of Meckel’s cartilage form the basis of major portions of two ear ossicles. Deficient development of the pharyngeal arches results in syndromes that are identified according to the arch involved. The syndromes become rarer as the number of the arch increases. Severe first-arch anomalies are: Agnathia, Synotia, Microstomia.       "body": "Executive summary\nBlockchain is a distributed ledger that is broadly discussed as a technology with huge  innovation potential in all areas of financial services. To date, it is largely in the banking  arena where blockchain use cases have been identified. However, the blockchain technology also offers potential use cases for insurers that include innovating insurance  products and services for growth, increasing effectiveness in fraud detection and pricing,  and reducing administrative cost. In these application areas insurers could address  some of the main challenges they are facing today – such as limited growth in mature  markets and cost reduction pressures.Implementation of blockchain has a long-term horizon as it depends on network effects  as well as on defining the regulatory conditions. Also, before initial implementation  steps are taken, the benefits and limitations of the technology need to be fully under- stood. Considering all of this, now is the best time for the insurance sector as a whole  and for individual insurance players to further investigate the blockchain technology  and its potential.\nQuite a few voices are calling the emerging blockchain technology the greatest revolu-tion since the advent of the Internet. In 2009, Bitcoin’s implementation of blockchain as the backbone for digital currency transactions was considered experimental and obscure. About five years later, financial institutions and central banks, along with VC started showing serious interest in applying blockchain beyond Bitcoin, and consortia such as R3 were set up. At the same time, investments in blockchain-related start-ups across industries have quickly grown to more than USD 800 million in 2014/15. The McKinsey Panorama FinTech database currently registers over 200 blockchain-related solutions, of which about 20 provide use cases for insurers that go beyond payment transactions – either as specific applications or as base platforms.\n Finally, even traditional insurance companies, such as  AXA and Generali, have started to invest in blockchain applications and Allianz has just recently announced its successful pilot of a blockchain-based smart contract solution to automate catastrophe swap transactions.\n\n1. What a blockchain is\n A blockchain is a distributed register to store static records and/or dynamic transaction data without central coordination by using a consensus-based mechanism to check the validity of transactions. As the Bitcoin backbone, blockchain was the first-ever solution to the double-spending problem that does not require a central administrator or clearing agent.\n It is thus well suited for applications requiring transparency on records with a permanent time and date stamp, such as titles, document histories, and notary services.\nWhile implementation spans different layers – from infrastructure to generic platforms to specific applications – a blockchain solution, without requiring central coordination, generally builds on a set of four characteristics:\nDecentralized validation.\n New data is packed into blocks that can only be added to the blockchain after consensus is reached on the validity of the action, e.g., a ticket seller is the legitimate owner of a ticket – see illustration below. This allows participants to place trust in their transactions even in the absence of a central authority, thus enabling disintermediation. Validation may require heavy computing power, which is provided by the processors in the computers of participants in the blockchain network, making it difficult for hackers to simulate and manipulate the validation. Alternative approaches to validation can be implemented, depending on the implementation’s targeted use case(s), e.g., to make it more scalable.\n\nRedundancy.\n The blockchain is continuously replicated on all or at least a group of nodes in the network. As a result, no single point of failure exists.\nImmutable storage.\n Each stored block is linked to its previous block in the chain, making it almost impossible for hackers to subsequently change blocks, as they would have to manipulate any succeeding block plus the majority of their replications. Further more, data is registered in the blockchain by creating a digital fingerprint using hash functions with a date and time stamp. Any attempt to change data will be apparent, because the new digital fingerprint will not match the old one. Combined with the chaining of blocks, the stored data becomes immutable, and full transparency on the history of transactions is reached.\nEncryption.\n Digital signatures based on pairs of cryptographic private and public keys put network participants in a position to authenticate which participant initiated a trans-action, owns an asset, signed a (smart) contract, or registered data in the blockchain.\n\nWith these characteristics, in general, blockchains can be used as a system of static record keeping (e.g., for land titles), and as a dynamic registry for the exchange of assets and payments as well as for the verification of dynamic information (e.g., tickets).  They are also a platform for smart contracts, which are small programs running on a blockchain and initiating certain actions when predefined conditions are met.\n\n2. Blockchain’s potential use cases for the insurance industry\n\nIn the banking industry, several blockchain use cases are currently being implemented, ranging from customer-facing payment technology to trading and exchange services.\n While the insurance industry (in terms of technology adoption) lags behind banking, it is nevertheless uniquely positioned to benefit from blockchain technology. Blockchain can address the competitive challenges many incumbents face, including poor customer engagement, limited growth in mature markets, and the trends of digitization. In the following, we outline the most promising insurance-related use cases in three categories: enabling growth, increasing effectiveness, and reducing cost by automating key processes.\nInnovate products and services for growth\nWe see three ways in which blockchain can facilitate growth for insurers: improving customer engagement, enabling cost-efficient product offerings for emerging markets, and enabling the development of insurance products related to the Internet of Things. Fundamental to the potential that blockchain offers in these areas is its usage as a dis-tributed and reliable platform for customer-controlled personal data, peer-to-peer (P2P) insurances, and smart contracts.\nCustomer engagement.\n An important lever for improving customer engagement through blockchain lies in the area of personal data. Customers’ fears about losing control of personal data as soon as it is handed over to a company and their frustration with the need to repeat data entry processes can be addressed by a customer-controlled block-chain for identity verification (see KYC use case) or medical/health data.Personal data does not need to be stored on the blockchain; it remains on the user’s personal device. Only its verification, e.g., through a doctor, and related transactions (e.g., an examination that has taken place on a certain date) are registered in the block- chain. Here, scale is key to reaping the benefits of blockchain as it requires a sufficient number of parties involved to reuse the verified data.\n\n“Speed up and simplify the onboarding of new customers” – the KYC use case\nStart-ups like\nTradle are working on blockchain solutions for know-your-customer (KYC) data. With KYC, the customer grants a company access to identity data when neces-sary for a contract closure. Once the KYC profile is verified, a customer can forward the verified identity data to other companies for different contracts with the same tool, avoiding the need to repeat the full identification and verification process, thus speed-ing up and increasing efficiency in the onboarding of new customers.\n\nBlockchain can also improve customer engagement by providing a greater degree of transparency and perceived fairness of tariffs and claims handling. For example, the start-up\n InsureETH\n demonstrated a P2P flight insurance policy built on blockchain with smart contracts. These smart contracts initiate payouts for insured flight tickets when cancellations or delays are reported from verified flight data sources (via so called “oracles” for making external sources usable for smart contracts in the blockchain). While P2P insurance as a business model is already being offered using standard tech-nology, blockchain makes it even more transparent and trustworthy for consumers as no central authority controls its operation. For the provider, it is a tool to widely automate P2P insurance operations.Whether they are used as part of P2P insurance or not, smart contracts on top of a block-chain offer several benefits: they enable automation of claims handling, they are a reliable and transparent payout mechanism for the customer, and they can be used to enforce contract-specific rules. For example, in the case of a car accident, a smart contract can ensure that the claim is only paid out if the car is repaired in a garage preferred and pre- defined by the insurer. Although such programs could also be implemented without blockchain, a blockchain-based smart contract platform could provide unique benefits. Not only does it deliver an increased degree of transparency and credibility for customers due to decentralization as well as automation of reconciliation and verification of trans-actions, but it also provides substantial network effects – either in the case of P2P insurances or when several parties are using it that would not be able or willing to do this with a centralized platform.\n\nIncrease effectiveness in fraud detection and pricing\n\n An estimated 5 to 10 percent of all claims are fraudulent. According to the FBI, this costs US non-health insurers more than USD 40 billion per year. To more effectively detect identity fraud, falsified injury or damage reports, etc., blockchain can be used as a cross-industry, distributed registry with external and customer data to: \n-Validate authenticity, ownership, and provenance of goods as well as authenticity of documents (e.g., medical reports) \n-Check for police theft reports/claims history as well as a person’s verified identity and detect patterns of fraudulent behavior related to a specific identity \n-Prove date and time of policy issuance or purchase of a product/asset \n-Confirm subsequent ownership and location changes.However, to achieve blockchain-specific benefits from these applications beyond what is possible with traditional database solutions and existing forms of cooperation – e.g., via industry associations –, intensive cooperation between insurers, manufacturers, custom-ers, and other parties is necessary. Estás besando a tu pareja y acariciándose mutuamente. En ese momento decides que es hora de penetrarla, empiezas a hacerlo despacio y al inicio notas que tienes cierto grado de control.\n\nSin embargo después de 30 o 60 segundos tu excitación se empieza a disparar, escuchas los gemidos de tu pareja y no aguantas, pierdes el control y terminas eyaculando. Si no tienes pareja actualmente no te preocupes, que también te daré algunos consejos al respecto.\n\nAntes  es muy importante aclarar algunos conceptos que necesitas saber:\n\nLa escala de la excitación\n\nPara poder entrenar nuestro cuerpo es necesario conocerlo primero. Necesitamos saber qué nos excita y qué no, así como reconocer las diferentes señales que nos da nuestro organismo. Esto nos permitirá medir el grado de excitación en el que nos encontramos para así poder hacer algo al respecto.\n\nLa respuesta sexual humana está dividida en tres fases de acuerdo a los expertos: excitación, meseta, orgasmo.\n\nLa fase de la excitación es la fase en la que surge el deseo sexual y obtenemos una erección.\n\nLa fase de meseta es la fase en la que estamos sintiendo gran placer y nuestra respiración, ritmo cardiaco y tensión muscular aumentan.\n\nCuando la fase de meseta llega a su punto más alto entonces inicia la fase del orgasmo, en la cual ocurren contracciones en la zona ano-genital y finalmente eyaculamos.\n\nLa fase del orgasmo se divide en dos sub fases:\n\nEl punto de no retorno es el momento en el que se empiezan a acumular fluídos seminales en la próstata, listos para ser expulsados. Este es el momento en el que tú sabes que vas a eyacular sin importar qué es lo que pase. Detener la eyaculación en este punto es casi imposible. Esto es conocido como la fase de carga o la fase de emisión.\n\nDespués del punto de no retorno sigue la fase de la eyaculación que consiste en varias contracciones musculares involuntarias en el área genital, las cuales provocan la expulsión del semen.\n\nEl inconveniente que tienen los eyaculadores precoces es que no logran mantenerse en la fase de meseta por mucho tiempo y además tienen dificultad para reconocer cuándo se acercan al punto de no retorno.\n\nPara simplicar las cosas, quiero que te imagines la excitación como una escala que va del 1 al 10.\n\nEn esta escala, el 0 indicaría que no hay excitación. El 1 indicaría que empieza a haber un poco de excitación e inicia el proceso de erección. Conforme va aumentando la escala el nivel de placer y excitación aumenta. Por ahí del número 5 el metabolismo se incrementa, causando que la respiración y el ritmo cardíaco se aceleren un poco. El nivel 6 y 7 se sienten muy bien y surgen grandes oleadas de placer. El nivel 8 y 9 son increíbles, el placer es intenso, el corazón y la respiración están bastante acelerados, no hay razón ni motivación alguna para detenerse. El nivel 9.9 es el punto de no retorno. Finalmente el 10 es el momento de la eyaculación.\n\nUn eyaculador precoz progresa del nivel 5 al nivel 10 de manera demasiado apresurada. Lo que queremos lograr es entrenarnos para así poder mantenernos en los niveles 5, 6, 7, 8 y 9 por períodos indefinidos de tiempo. Para lograrlo necesitamos conocernos y saber en qué posición de la escala nos encontramos en un momento determinado.\n\nCon suficiente práctica es posible mantenerse en los niveles 8 y 9, disfrutando así de grandes olas de placer sin interrupción.\n\nAhora, antes de proceder al  entrenamiento individual (sin pareja) me gustaría hablar acerca de un par de herramientas naturales que son de suma importancia.\n\n¿Recuerdas lo que sucede con tu cuerpo cuando tienes un orgasmo? Tu ritmo cardíaco aumenta, tus músculos se tensan, pero lo más importante, tu respiración se acelera.\n\nLa respiración y la relajación van de la mano. Si te encuentras relajado y sin preocupaciones, tu respiración va a ser lenta y tranquila. Si te encuentras agitado, nervioso o excitado, tu respiración va a ser rápida.\n\nLa ventaja de la respiración es que es un proceso tanto inconsciente como consciente. A ti nadie te enseñó a respirar, es algo que sucede sin pensarlo, de manera automática. Sin embargo, a pesar de ser un proceso automático también tienes el poder de controlarlo.\n\nSi quieres puedes empezar a respirar más rápido ya mismo o aguantar la respiración por 10 segundos. Esta habilidad de poder controlar la respiración es extremadamente beneficiosa ya que nos va a permitir poder relajarnos más.\n\nTodas las prácticas espirituales, deportes y ejercicios de relajación dan gran importancia a la respiración. La meditación, el yoga, las artes marciales, la natación. Respirar bien en estas actividades es fundamental.\n\nEl sexo no es la excepción.\n\nTu primera misión será utilizar la respiración como una herramienta para lograr mantenerte relajado y en control de tu cuerpo. \n\nEstar consciente de la respiración durante el sexo tiene 2 ventajas:\n\n1)\tAl respirar de manera correcta, vas a disminuir tu ritmo cardiaco y la tensión de tus músculos. Te mantendrás más tiempo en modo parasimpático y así evitarás que tu excitación se salga de control.\n\n2)\tAl estar enfocado en tu respiración, tu mente se tranquilizará. En lugar de ser un mar en medio de una tormenta, lleno de pensamientos eróticos y preocupaciones, tu mente será como un lago tranquilo, donde la paz y la relajación reinarán.\n\nA continuación te voy a enseñar cómo debes de respirar. Este es quizá uno de los métodos más efectivos, sin lugar a dudas.\n\nSi no aprendes a respirar bien, NUNCA serás capaz de controlar tu eyaculación precoz. \n \nLo primero que tienes que hacer es aprender a respirar con tu diafragma. El diafragma es un músculo que se encuentra en la parte superior de tu abdomen, abajito de las costillas.\n\nCuando contraes este músculo, la cavidad torácica se expande, lo cual ocasiona que entre aire a tus pulmones para llenar el vacío resultante. Cuando el músculo se relaja y vuelve a su posición normal entonces los pulmones se contraen y el aire se expele.\n\nRespirar utilizando el diafragma es muy fácil.\n\n1)\tSiéntate en una silla.\n\n2)\tPon tu mano sobre tu abdomen.\n\n3)\tRespira profundo de tal forma que sientas que tu abdomen se llene, como si fuera un globo llenándose de aire. Tu abdomen debe de estar relajado y no tenso.\n\nSi no sientes que tu abdomen se llena entonces no estás respirando con tu diafragma. Continúa practicando hasta que lo domines y te salga natural y sin esfuerzo.\n\nUna vez que aprendiste a respirar con tu diafragma entonces tienes que acostumbrarte a respirar despacio y consistentemente. Si respiras rápido entonces vas a crear más tensión en tu cuerpo y vas a aumentar tu ritmo cardíaco. Así que respira lento, consistente y con una pequeñita pausa entre cada respiro.\n\nToma en cuenta que esto es algo que debes de hacer desde el puro inicio de la interacción sexual con tu pareja. No empieces a respirar bien una vez que estás teniendo sexo, sino que hazlo desde que están besándose y tienen la ropa puesta. Esto es muy importante.
